胸腰椎爆裂骨折椎管内骨块复位的生物力学与解剖学研究 被引量:30

摘要 目的 :研究胸腰椎爆裂型骨折椎管内骨块复位的解剖学因素及相关的生物力学分析。方法 :采用新鲜胸 11(T11)~腰 3 (L3 )段脊柱 ,制备成腰 1(L1)爆裂骨折模型 ,记录椎间盘切断而后纵韧带完整 ,后纵韧带切断而椎间盘完整 ,后纵韧带和椎间盘均完整三组在恒速纵向拉伸过程中椎管内骨折块复位过程中的位移变化 ,描记出纵向拉伸载荷与骨块复位百分率之间的关系曲线。结果 :纵向拉伸载荷与骨块复位百分率之间的关系曲线呈“S”型。在复位前期曲线上升平缓 ,所需纵向拉伸载荷按Ⅰ、Ⅱ、Ⅲ组的顺序递增。在复位期曲线急剧上升 ,纵向拉伸载荷及复位百分率均按Ⅰ、Ⅲ、Ⅱ组的顺序递增。在后复位期第Ⅲ组仍有明显的复位曲线存在。结论 :纵向拉伸载荷对椎管内骨折块的复位有重要影响 ,完整的椎间盘纤维环及后纵韧带在骨折块的复位过程中起决定性作用。 Objective: To study the anatomic factors of the reduction of fragment in the spinal canal for thoracolumbar burst fracture and the related biomechanic relationship. Methods: Using burst fracture model of L 1 made by fresh thoracolumbar spine (T 11~L 3), the change of the narrowest place in the spinal canal under axial distraction was drew. At the same speed for group one with the posterior longitudinal ligament (p11)being intact and the fibrous annulus being cut off, group two with the fibrous annulus being intact and the pll being cut off, group three with both being intact, the curve of the relationship between the reduction percentage of fragment and the axial distraction was drawn. Results: Curves of the relationship between the reduction percentages of fragment and the axial distraction were 's' shaped . Curves ascended slowly and the axial distractions rised progressively on the sequence of Group Ⅰ,Ⅱ, Ⅲ in pre-reduction period .Curves ascended acutely in reduction period. The axial distraction and the reduction percentages rised on the sequence of Group Ⅰ, Ⅲ and Ⅱ.There was still a significant reduction curve during the post-reduction period. Conclusion: The axial distraction takes an important role on the reduction of fragment in the spinal canal for the thoracolumbar burst fracture, and intact pll and fibrous annulus take a key role in the course of the reduction of fragment.
