摘要
目的:探讨血透患者血清骨钙素(BGP)、降钙素(CT)、甲状旁腺素(PTH-M)、骨密度(BMD)及β2-微球蛋白(β2-MG)等的变化规律及其相互关系的临床价值。方法:用放射免疫(RIA)法测定40例慢性肾衰(CRF)血透患者透析前、后BGP、CT、PTH-M、及β2-MG等在血清中的变化,分析其间及其与部分生化指标之间的关系。其中测定了部分患者的BMD。结果:血透患者透析前BGP、CT、PTH-M及β2-MG均较对照组显著升高,透析后均较透析前显著降低;β2-MG与BGP、CT分别呈显著性正相关。19例血透患者BMD较对照组显著降低。结论:测定血清中BGP、CT、PTH-M及β2-MG等浓度及BMD,对于了解血透患者病情、疗效及估计预后等有一定临床价值。
Objective: To explore the clinical value in the variation law and the relationships among BGP, CT, PTH-M and β2-MG in serum of 40 patients with CRF ante- and post-haemodialysis and BMD of them. Methods: The relationships among BGP, CT , PTH-M, β2-MG, AKP,BUN and Cr were analysed by testing their levels in serum of CRF patients with RIA; BMD of the lumbar spine in 19 of 40 patients with CRF was measured by dual-energy X-ray absorptiometry(DEXA). Results: The levels of BGP, CT, PTH-M, β2-MG, AKP, BUN and Cr in serum of patients with CRF were higher than that of the controls. And that of patients with CRF in post-hemodialysis was very lower than that of those in ante-hemodialysis .The level of β2-MG in serum of patients with CRF was positively correlated with that of BGP and CT respectively. BMD of 19/40 patients with CRF were higher than that of the controls. Conclusion: There is a significant clinical value in testing β2-MG, BGP, PTH-M and CT concentrations in serum for analysing patient's condition, assessing the treatment effect and estimating prognosis of patients with CRF.
