滤器预防肺栓塞:滤器血栓及相关因素分析 被引量:15

摘要 目的 对腔静脉滤器使用后出现的滤器血栓进行研究 ,分析其发生的相关因素。方法 对 73例安放滤器3~ 86个月的病人 ,用彩色多普勒和增强CT扫描检查有无滤器血栓。共安放滤器 73支 ,其中LGM滤器 (VTF) 5 5支 ,钛制Greenfield滤器 (TGF) 6支 ,Simon镍钛合金滤器 (SNF) 12支。结果  9只VTF、1只TGF内有血栓。安放滤器是绝对或相对适应证、深静脉血栓上端位置在髂静脉以上或以下、安放滤器后抗凝少于或多于 3个月 ,滤器血栓的发生率与适应证 (绝对或相对 )、安放滤器后抗凝无统计学显著性差异 ,1例滤器放置后再发PE。结论 滤器预防PE安全、有效 ,但是由于长期安放后发生的滤器血栓与再发PE有关 ,使用应严格掌握适应证。 Objective To define the frequency of thrombosis in caval filters after long-term placement of caval filters and identify related factors. Methods Seventy-three caval filters (55 VTF, 6 TGF, 12 SNF) were placed in the inferior vena cava. The thrombosis in filters were detected by color-coded duplex sonography and contrast-enhanced computer tomography (CT). Other follow-up examinations included clinical evaluation and plain radiographs. The mean observation time was 28 months. Results Thrombosis happen in filters were seen in 16.4% (9/55) of the patients with VTF and in 16.7% (1/6) of those with TGF. It was not related to indications (absolute or relative) and anticoagulant therapy after filter placement. Recurrent pulmonary embolism was only noted in 1 patient with a thrombosis of VTF. Conclusion The implantation of vena cava filters for prevention of pulmonary embolism is a effect and convenient method. The vena cava filter should only be inserted in strict selected patients, for the possibility of filter thrombosis after long term placement.
