脑脊液CPK,LDH,AST,α-HBD和ALT对中枢神经系统疾病的诊断意义 被引量:10

摘要 目的 :对 1 0 7例中枢神经系统疾病患者脑脊液(CSF)CPK ,LDH ,AST ,α HBD和ALT活性进行测定 ,探讨各疾病酶活性变化 .方法 :被检测CSF ,离心取上清当日测定 .采用酶法测定CPK ,LDH ,AST ,α HBD和ALT ,试剂由长征公司提供 .仪器用日立 71 70自动生化分析仪 ,化学参数以试剂盒说明书为准 .结果 :CSFCPK以急性颅脑损伤最高 ,与其他组相比较有非常显著性差异 ;以结核性脑膜炎为最低 .急性颅脑损伤与化脓性脑膜炎、病毒性脑膜炎与脑肿瘤相比较 ,有非常显著性差异 (P <0 .0 1 ) .脑血管疾病与对照组相比 ,有显著性差异 (P <0 .0 5 ) .结核性脑膜炎与对照组相比无显著性差异 (P >0 .0 5 ) .CSFLDH ,α HBD活性在中枢神经系统炎症时均明显升高 ,以急性颅脑损伤升高最显著 ,与其他组相比均有非常显著性差异 (P <0 .0 1 ) ;化脓性脑膜炎与脑肿瘤LDH和α HBD活性均差异非常显著 (P <0 .0 1 ) ;病毒性脑膜炎与脑血管疾病均差异非常显著 (P <0 .0 1 ) ;脑血管疾病与结核性脑膜炎虽然均高于对照组 ,但均差异不显著 (P >0 .0 5 ) .CSFAST ,ALT在几组脑病中活性均增高 ,以化脓性脑膜炎、脑血管疾病、急性颅脑损伤、病毒性脑膜炎时增高显著 ,结核性脑膜炎最低 ,但均高于对照组 (P <0 .0 5 ) .结论 :提示CKP ,LDH 。 AIM: To probe the changes of enzyme activity in all kinds of brain diseases by measuring the activity of cerebral spinal fluid CPK, LDH, AST, α HBD and ALT in 107 cases of central nervous system diseases. METHODS: CSF was measured and centrifuged to get supernatant and it was measured within 24 hours. Enzyme method was employed to measure CPK, LDH, AST, α HBD and ALT in CSF. RESULTS: CPK in CSF in acute brain's hurt was the highest, and compared with the other groups it had significant difference, while CPK in CSF in tuberculous meningitis was the lowest. By comparing the acute brain's hurt with purulent meningitis, and viral meningitis with cerebroma, there were significant differences ( P <0.01). By comparing cerebrovascular disease with control group, there were obvious differences ( P <0.05). There were no obvious differences between tuberculous meningitis and control group ( P > 0.05). The activity of CSF LDH and α HBD rose obviously when central nervous system inflammation occurred, and the activity rose most significantly in the acute brain hurt, and there were obvious differences compared with the other groups ( P <0.01).Comparing purulent meningitis with cerebroma, there were obvious differences in the activity of LDH and α HBD ( P <0.01) and there were also obvious differences between viral meningitis and cerebrovascular disease in the activity of LDH and α HBD ( P < 0.01) . Though the activity of LDH and α HBD in cerebrovascular disease and tuberculous meningitis was higher than that of control group, the differences between them were slight ( P > 0.05). While the activity of AST and ALT in CSF rose in all these groups of brain diseases, the activity of purulent meningitis, cerebrovascular disease, acute brain's hurt and viral meningitis groups rose obviously, and the activity of tuberculous meningitis group changed least, but all of them were higher than the control group ( P <0.05). CONCLUSION: It is suggested that the measurement of the activity of CKP, LDH, AST, and α HBD provides important bases for diagnoses of a variety of brain diseases, and it is also of value in evaluating the extent of damage and prognosis of brain diseases.
