摘要
目的 :拟通过对椎 -基底动脉供血不足(VBI)病人进行经颅多普勒超声 (TCD)、颈部血管的彩色多普勒血流显像 (CDFI)检查 ,探讨二者对该病的诊断价值与限度。方法 :对临床诊断VBI病人 42例在发病后 3d之内进行TCD、CDFI检查 ,并详细记录检查结果 ,同时与14例健康老年人进行对照分析。结果 :观察组TCD、CDFI血流动力学变化与对照组相比 ,主要表现为半径减小 ,血流速度减慢 ,脉动指数(PI )增高。VBI病人TCD检测阳性率为64 .2 9% ,特异度为 92 .86%。CDFI检测阳性率为 90 .48% ,特异度为 85 .71%。TCD与CD FI相结合检测阳性率明显提高 ,达 97.62 % ,特异度为 85 .71%。结论 :TCD与CDFI相结合可以作为诊断VBI的首选检查手段 ,且因其无创、价廉、便捷在临床上可广泛应用和推广。
Objective:To explore the objective and precise diagnosis method for the Vertebrobasilar Insufficiency (VBI) in the elderly. Methods:The TCD and CDFI of 42 cases with VBI with 3 days from the last onset of symptoms were investigated and compared with control groups. Results:In the VBI group, the sensitivity of TCD is 64.29%, CDFI is 90.48%. The combination of TCD and CDFI make the sensitivity improve 97.62%. Conclusion:The combition of TCD and CDFI improve the sensitivity of diagnosis of VBI. It is a good and valuable method to extend in clinical application.
