摘要
目的 评价乳腺癌血流特征在彩色多普勒超声诊断中的价值。方法 用CDFI、PDI和 3D DMS检查 171例乳腺肿块 ,定量分析血流灌注量、Vmax、RI、血管数量。结果 乳腺癌相对灌注为 ( 8.2 0 0± 0 .0 0 1) % ,良性肿块 ( 2 .40 0±0 .0 0 2 ) % (P <0 .0 1) ;乳腺癌Vmax为 ( 2 5 .68± 1.11)cm/s,良性肿块 ( 6.10± 0 .2 2 )cm /s (P <0 .0 0 1) ;乳腺癌PI为 1.2 9±0 .3 6,良性肿块 0 .83± 0 .2 2 (P <0 .0 1) ;乳腺癌RI为 0 .7± 0 .0 10 ,良性肿块 0 .49± 0 .0 11(P <0 .0 1) ;血管数量乳腺癌为1.810± 0 .0 2 6,良性肿块 0 .98± 0 .10 0 (P <0 .0 0 1) ,以上各组数字均显示乳腺癌明显高于良性肿瘤。CDFI、PDI和 3D DMS诊断乳腺癌的敏感性为 96.9% ,特异性为 93 .8% ,准确性为 96.8%。结论 超声CDFI、PDI和 3D DMS对早期乳腺癌诊断具有独特价值。
Objective To evaluate the diagnositic value of blood flow characteristics of color Doppler ultrasound in breast cancer. Methods In 171 patients, masses of breast were investigated with CDFI, PDI and 3D-DMS. Vmax, PI, RI, vessel numbers and blood flow fill were analyzed quantitatively. Results Blood flow fill, Vmax, PI, RI and vessel numbers in breast cancer were (8.200±0.001)%, (25.68±1.11)cm/s, 1.29±0.36, 0.7±0.010 and 1.810±0.026 respectively. They were higher than those in benign breast group (2.400±0.002)%, (6.10±0.22)cm/s, 0.83±0.22, 0.49±0.011 and 0.98±0.100 (P<0.01, P<0.001, P<0.01, P<0.01 and P<0.001 respectively). The diagnostic sensitivity, specificity and accuracy of CDFI, PDI and 3D-DMS for breast cancer were 96.9%,93.8%,96.8% separately. Conclusion Ultrasonic CDFI, PDI and 3D-DMS have a singular value of early diagnosis of breast cancer.
