摘要
目的 回顾分析超声检出各类畸形的孕周及使用方法,得出各类畸形最适宜的检查孕周及超声检查方法,以求及时有效准确地检出胎儿畸形。 方法 于1982年7月至2002年6月对孕13周至41周79786例孕妇进行二维实时超声产前畸形筛查:于1990年7月至2002年6月对孕19周至39周1752例孕妇使用彩色多普勒及超声心动图进行胎儿心脏超声检查;同时对前十年和后十年的二维超声检查结果及应用不同超声类型检查胎儿心脏结果进行比较分析。 结果 前十年间胎儿畸形的检出率为49％(160/327),后十年间胎儿畸形的二维超声检出率为63％(264/422),胎儿心脏异常的彩色多普勒超声心动图捡出率为71％(37/52),所检出之心脏异常中,首先由二维超声怀疑者为73％(27/37)。 结论 超声工作者经验的积累及仪器的改善,将有助于胎儿异常检出准确率的提高,在合适的孕周选择适当的超声检查方法既可及早发现胎儿异常,又可正确合理地使用医疗资源,这更适合我国国情。
Objective The sorts of fetal malformations were examined by different ultrasonography & at distinct gestational age had been analyzed, to seek for certain changes in ultrasonography at suitable gestational age, as well as to effectively find congenital of fetal malformations in time. Methods 79786 cases of fetuses had been examined by two-dimensional real time ultrasound in 13-41 gestational weeks from July 1982 to June 2002. Comparison of the results was made between the two decades; 1752 cases of the fetuses had been examined with color Doppler & echocardiography in 19-39 gestational weeks during the period of July 1990 to June 2002, The results of the Doppler had been compared with that of the two-dimensional ultrasound. Results In the former ten years, the rate of finding congenital fetal malformations was 49% (160 / 327) ; while that during the latter ten years was 63%(264 / 422). The rate of fetal heart malformations detected was 71% (37 / 52) , while that first detected by two-dimensional ultrasound examination was 73%(27 / 37). Conclusions The accumulation of experience & the improvement of examining instruments will be helpful in raising the rate of accuracy in detecting fetal malformations. Appropriate ultrasonograhy examination during right gestational age can either find congenital malformation of fetus in time or reasonably use the resources of medicine, which fits the condition of our country.
