摘要
目的 :研究经颅彩色双功超声显像 (TCCD)方法学及诊断颅内占位性病变和脑血管畸形的可靠性。材料和方法 :颅内占位性病变 (ISOL)及脑血管畸形 (CVM)共 73例 ,探头频率 2 .0~ 3 .5MHz ,彩色多普勒 (CDFI)频率 2 .0MHz。颞窗是主要声窗 ,探头行横切、纵切、斜切扫查。结果 :ISOL及CVM共 66例 ( 90 .4% )被检出 ,CVM包括动静脉畸形 (AVM)及动脉瘤 (aneurysm)。ISOL 6例及CVM 1例 ( 9.6% )超声未检出的原因与病变类型及声窗无关 ,主要与病变位置有关。结论 :TCCD可作为对诊断与鉴别诊断AVM及动脉瘤的较好方法 ,对ISOL可作为辅助诊断及随诊的方法。
Purpose: To study the methodology and reliability of transcranial color-coded real-time duplex sonography(TCCD). Materials and Methods: Intracranial space-occupying lesions (ISOL) and cerebral vascular malformations (CVM) in 73 cases were examined with TCCD at 2D probe frequency of 2~3.5MHz and color doppler 2 MHz. Through acoustic window mainly at temple, sagittal?coronal and obliqueal images were obtained. Results: In 66 cases (90.4%) were ISOL or CVM found including 10 cases of AVM and 3 aneurysm. TCCD failed in 6 tumors and 1 AVM (9.6%). It was suggested that the failure was not related to the types of lesions and acoustic window, but to the site of lesions. TCCD could not detect completely feeding arteries and back flow veins of AVM. Conclusion: TCCD was a method of value in diagnosis and differential diagnosis of AMV, and a useful tool assisting diagnosis and follow up of ISOL.
