摘要
目的 :探讨上皮性卵巢癌癌旁囊肿和良性囊腺瘤P5 3的表达情况。方法 :应用免疫组化技术 ,检测 5例正常卵巢 ,2 1例卵巢良性囊腺瘤和 4 0例上皮性卵巢癌组织中的 (其中 1 0例有癌旁囊肿 )P5 3蛋白表达 ;然后检测同一卵巢癌组织中恶性区域 (囊腺癌 )和癌旁囊肿P5 3蛋白的表达情况。结果 :P5 3蛋白主要表达于上皮性卵巢癌中(1 9/ 4 0 ,4 7.5 0 % ) ,而良性囊腺瘤和正常卵巢中未见表达 ;1 0例癌旁囊肿P5 3蛋白表达阳性者 5例 ,与良性囊腺瘤P5 3表达相比 ,2者差异有统计学意义 (P =0 .0 0 1 ) ,囊腺癌与其癌旁囊肿的P5 3表达完全一致 ,即囊腺癌P5 3蛋白阳性者其癌旁囊肿P5 3蛋白表达亦阳性 ,反之亦然。结论 :P5 3的表达有助于卵巢良恶性病变的鉴别。卵巢癌癌旁囊肿不同于卵巢良性囊腺瘤 ,2者在分子遗传学方面有着本质区别 。
Aim:To investigate the P53 expression in adjacent cyst of ovarian carcinoma and solitary cystadenomas. Methods: By using immunostaining techniques, P53 protein was detected in 21 solitary ovarian cystadenomas, 5 normal ovaris, and 40 epithelial ovarian carcinomas(10 with adjacent cyst). Results: P53 proteins expression was observed in ovarian carcinomas (19/40, 47.50%) but none in solitary ovarian cystadenomas and normal ovaris. P53 protein were positively expressed in 5 morphologically benign cysts out of 10. The expression of P53 protein between solitary cystadenomas and benign cysts was significantly different ( P =0.00148 ). Furthermore, the expression of p53 protein in morphologically benign cysts was concordant with that in cystadenocarcinomas. That was, any cystadenocarcinoma which was P53 protein positively stained, its morphologically benign portion was also P53 protein positively stained and vice versa.Conclusions: Expression of P53 protein can be used to distinguish ovarian malignant tumors from benign ones. Morphologically benign cysts adjacent to ovarian carcinomas are not the same with ovarian solitary cystadenomas. Histologically benign cysts adjacent to ovarian carcinomas may have a tendency to carcinogenesis that is not present in solitary cystadenomas.
