多种肿瘤标志物在肺癌细胞上的表达及临床应用 被引量:5

The expression of multiple tumor markers on solid lung cancer and their potential clinical application

摘要 目的 研究CD4 0、主要组织相容性抗原 I(MHC Ⅰ )、Fas、细胞粘附分子 1(ICAM 1)、上皮细胞生长因子受体 (EGFR)、P糖蛋白 (PGP)、肺相关蛋白 (LRP)、bcl2、突变型P53 和细胞倍体分析等多个指标在实体肺癌细胞上的表达及其临床意义 ,旨在寻找对肺癌诊断、预后判断及评价发生发展的新指标。方法 通过流式细胞术检测 6 8例实体组织标本上各种指标的表达情况 ,进而分析这些参数与肺癌的临床关系。结果 肺癌组的EGFR、PGP和P53 的表达明显高于对照组 ;CD4 0、EGFR和CD5 4在已转移的原发性肺癌组织中的表达明显高于未发生转移者 (P <0 0 0 5 ) ;这些肿瘤标志物与肺癌组织类型之间无明显相关性 (P >0 0 5 ) ;所有多倍体均为原发性肺癌组织 ,原发性肺癌组织中 86 1%为多倍体 ,良性肺占位和正常肺组织均为二倍体。在生存期超过 5年者 (7例 )中 ,以CD4 0低表达者 (表达率 <80 % )和PGP不表达者 (表达率 <10 % )为主 ;在生存期不足 5年者 (8例 )中 ,以CD4 0高表达者和PGP表达者为主。结论 多倍体的出现是肺癌的特异性指标 ;EGFR、PGP和突变型P53 有望成为诊断肺癌的监测指标 ;CD4 0、EGFR和CD5 4有可能成为判断肿瘤发生发展的有用指标 ;CD4 0和PGP有可能成为判断预后的指标。 Objective To try to find new tumor markers serving as diagnostic and prognostic markers and indicators of advanced disease, we studied expression of CD40, MHC-Ⅰ,Fas, ICAM-1,EGFR,PGP,LRP,bcl2,P 53 and cell cycle on human lung cancer and their clinical application. Methods We obtained 68 surgical biopsy specimens, the surface expression of CD40, MHC-Ⅰ,Fas, ICAM-1,EGFR,PGP,LRP,bcl2,P 53 and cell cycle on these tissues were determined by flow cytometric cell phenotyping analysis and PI staining assay. Results The expression of EGFR, PGP and P 53 in tumor group increased significantly more than that in control group ( P <0.05). In tumor group, the expression of CD40, EGFR and CD54 increased more significantly in tumor spreading group than no-spreading group ( P >0.05). There was no correlation between the expression of these markers and the histology of the lung cancer ( P >0.05). All of aneuploid cell cycles belong to primary lung cancers. 86.1% primary lung cancers were aneuploid cell cycles. Non-malignant lung tissue and normal lung specimens showed diploid cell cycles. In 7 patients who survived over 5 years, 71.4% patients were CD40-low expressing (<80%), 85.7% were PGP non-expressing (<10%), 71.4% were CD40-low expressing and PGP non-expressing. In 8 patients who survived less than 5 years, 87.5% patients were CD40-high expressing (>80%), 100% were PGP expressing (>10%), 87.5% were CD40-high expressing and PGP expressing. Conclusion Aneuploid cell is a specific marker for diagnosing lung cancer. EGFR, PGP and P 53 may be used to diagnose lung cancer. CD40, EGFR and CD54 may play a role in meta static spread, CD40 and PGP may serve as a prognostic marker.
