CACI系统由IBM-PC微机与输液泵组成。它按照药物的分布、结合、转化与排泄规律给药以达到控制血药浓度的目的。 实验分三组,第1组7例病人,应用本系统在静脉麻醉中控制硫喷妥钠10μg/me的恒定血药浓度;第2组10例病人,硫喷妥钠的血药浓度按临床需要调整;第3组10例病人,普鲁卡因控制在15μg/me恒定的血药浓度,三组均获得满意结果。 作者认为CACI系统的设计达到了以下目的:静脉麻醉中控制稳定的血药浓度,避免血药浓度的剧烈波动,预测血药浓度的变化,进而调节控制静脉麻醉中的血药浓度。
A computer-controlled continuous infusion (CACT) system was assembled. Included in the system hardware was an IBM-PC computer connected to a YBS-11 computer controlled infusion pump via an interface. Software was written in the basic language. The CAGI system can rapidly attain and maintain a constant plasma concentration, as well as allow the anesthesiologist to change the plasma level of intravenous anesthetics according to distribution, combination, biotrasfomation and excretion of the drug. Themathematic model of the CACI system was based on pharmacokinetics and the concept of pasma drug efflux.Twenty-seven adult patients (three groups) undergoing a variety of surgical procedures under intravenuos anesthesia participated in the study. Group 1,seven patients recieved programmed thiopental infusion to achieve and maintain 10μg/ml constant plasma concentation. The average of 88 plasma concentrations was 10.09μg/ml in which the maximum variation was with the range of ±20% of the desired plasma drug concentration. Ten patients of gruop 2 recieved tiopentone at variable plasma levels controlled by anesthesiologists. There was an excellent correlation coefficient, r=0.8872 (p<0. 001) between measured and computer predicted plasma thiopental concentrations. In group 3 of ten patients, 15μg/ml of constant plasma procaine level was controlled by another programmed infution and a satisfactory result was obtained with 15.01±2.26μg/ml.These data indicate that the compute-assisted automated infusion is an easy way to provide constant plasma drug levels and avoid fluctuations. Also, the CACI system can predict plasma concentrations and permit anesthesiologists to change plasma drug levels of intravenous anesthetics according to clinical requirements
