压力导丝在冠状动脉介入治疗中的应用 被引量:5

摘要 目的 用压力导丝测定冠脉内压力 ,计算心肌部分血流储备 (FFRmyo)指导冠脉内支架优化置入及判定治疗效果。方法 介入治疗前经导引导管送压力导丝进入靶血管 ,使其传感器位于病变近端 ,使该处导管与压力导丝记录的压力相等 ,然后送压力导丝通过病变使传感器位于病变远端 ,同时记录导管和导丝压力 ,导丝测得的狭窄远端血管内的平均压与导管测得的主动脉的平均压比值即FFRmyo ,介入治疗后再次重复测定腺苷最大扩张冠脉后的FFRmyo。此项新技术限于设备国内仅极少数单位应用于临床。结果 本文首例患者应用此技术 ,介入前测得的FFRmyo为0 74 ,释放支架至病变处 ,冠脉内注入腺苷诱发最大充血相并持续记录压力及计算FFRmyo为 0 98。结论 压力导丝可更客观地选择介入治疗的适应症、指导支架优化置入及判定治疗效果 。 Objectives To evaluate the practicality and safety of intracononary pressure wire. Methods The myocardial fractional flow reserve(FFRmyo) is a new index of the functional severity of coronary stenoses that is calculated from aortic and distal coronary pressures measurements during maximal coronary hyperemia made during coronary arteriography. In one patient with acute coronary syndrome before and after stents were implanted, FFRmyo was used to evaluate whether optimum results were achieved. Results Optimum deployment was finially presented according to myocardial fractional flow reserve(FFRmyo). Conclusions The manipulation of pressure wire was similar to PTCA wire. It can replace PTCA wire with safety and reliability. FFRmyo can accurately reveal the severity of stenosis and function of coronary lesions. It can help the physicians choose suitable indications for interventional therapy and evaluate the effect. Intracononary pressure measurement can be used more cheaper and more rapid.
