摘要
目的 探讨NK/T细胞淋巴瘤 (NK/TCL)的临床病理学特征和免疫组化表型及TCRγ基因重排的检测 ,为诊断和鉴别诊断提供依据。方法 收集 30例NK/TCL及其临床资料 ,用免疫组化S P法标记LCA ,CD5 6 ,CD3,CD45RO ,CD2 0及CK抗体和聚合酶链式反应扩增方法检测 12例NK/TCL的TCRγ基因重排。结果 NK/TCL组织学特点为多形性淋巴细胞增生 ,大片的凝固性坏死和血管浸润 ;免疫组化表达LCA、CD3、CD45RO和CD5 6阳性 ,CD2 0、CK阴性 ;12例NK/T细胞淋巴瘤TCRγ基因重排的阳性率为 6 6 .7%(8/ 12 )。结论 NK/TCL是一组异质性肿瘤 ,结合其组织学特点和免疫组化表型可做出诊断 ;NK/TCL超过半数存在TCRγ基因重排 ,表明大部分为T细胞的单克隆性增生。
Objective To explore the clinicopathologic features and immunophenotypes of NK/T cell lymphoma(NK/TCL)and the significance of detection of rearrangement of TCRγgene in diagnosis and differential diagnosis of NK/TCL.Methods Thirty cases of NK/TCL were studied and immunohistochemical staining for LCA、CD56、CD3、CD45RO、CD20、CK were carried out with S-P method,12 cases were investigated for TCRγgene rearrangements by polymerase chain reaction(PCR)technique.Results Proliferation of pleomorphic lymphocyte,coagulation necrosis of massive tissue and vaso-inflitration were showed in NK/TCL;tumor cells were positive for LCA、CD3、CD45RO and CD56,while negative for CD20 and CK;The positive amplification of clonal TCRγ gene in 8 out of 12 samples(66.7%)from NK/TCL.Conclusion NK/TCL shows cytologic variability from case to case,can be diagnosed according to the histological feathers and immuno-phenotypes.More than half cases have TCRγ gene rearrangement in NK/TCL, which indicates more cases are monoclonal T-cell proliferation in NK/TCL.
