摘要
目的 研究系统性红斑狼疮 (SLE)外周血淋巴细胞上bcl-2抗原的表达及与疾病活动的相关性。方法 通过流式细胞仪 ,采用微量全血直接双标记免疫荧光染色方法 ,对 60例活动期SLE、3 0例类风湿性关节炎 (RA)及 3 0例正常人外周血CD3 + ,CD4+ ,CD8+ 淋巴细胞亚群上bcl-2抗原进行检测。结果 活动期SLE患者外周血CD3 + ,CD4+ ,CD8+ 淋巴细胞亚群上bcl-2抗原的表达较RA组和正常人降低 (P <0 0 0 1) ,bcl-2抗原的表达与SLE的活动性呈负相关 (r =-0 73 2 ,P <0 0 0 1) ;SLE患者中有 3 6例抗dsDNA抗体阳性 ,抗dsDNA抗体 ( +)患者组T淋巴细胞上bcl -2抗原的表达较抗dsDNA抗体 ( -)组更低 (P <0 0 5或P <0 0 1)。结论 SLE患者T淋巴细胞表面bcl -2抗原表达降低 ,导致淋巴细胞凋亡异常 ,刺激机体产生抗dsDNA等多种自身抗体 ,是SLE免疫功能紊乱的原因之一。
Objective To investigate the expression of bcl-2 antigen on T lymphocyte subsets and its relationship with production of anti-dsDNA antibody and activity of systemic lupus erythematosus (SLE). Methods bcl-2 antigen on T lymphocyte subsets of 60 active SLE patients, 30 rheumatoid arthritis (RA) patients and 30 healthy individuals were determined by using immunofluoresence double-staining method and flow cytometry. Results The expression of bcl-2 antigen on CD3 +, CD4 +, CD8 + T lymphocyte subsects of SLE patients was significantly lower than that of RA group and healthy group ( P <0 001). There was negative correlation between bcl-2 antigen on CD3 + subset and active index of SLE ( r=-0 732, P<0 001) . On the other hand, bcl-2 antigen expression on lymphocytes in patients with positive anti-dsDNA antibody was low, compared with that in patients with negative anti-dsDNA antibody (P<0 05 or p<0 01). Conclusion Expression of bcl-2 antigen on T lymphocyte subsets decreases in SLE patients and correlates with activity of SLE. Decreased expression of bcl-2 antigen on T lymphocyte subsets accelerates the apoptosis of lymphocytes and stimulates the production of autoantibodies, leading to immunologic disorder in SLE.
