摘要
目的 通过分析TCRVβ基因片段选择性扩增 ,了解白血病患者异基因外周血干细胞移植后T淋巴细胞的免疫重建及其在移植物抗宿主病 (GVHD)患者中的表达特点 .方法 采用RT -PCR扩增 10例移植后患者外周血的单个核细胞的TCRVβ 2 4个基因片段 ,分析其TCRVβ基因的表达情况 ,GVHD患者的PCR产物进一步经基因扫描分析确定T细胞克隆性 .结果 经 2 4个Vβ引物所分别进行的RT -PCR检测TCRVβ各亚家族基因的表达情况 ,发现 10例病人外周血与正常人表达 2 4个Vβ亚家族有明显的不同 ,病人的部分Vβ亚家族T细胞仍未能重建 ,仅表达 5~ 2 2个亚家族基因 ;9例GVHD患者外周血仅表达 2~ 8个TCRVβ亚家族 ,其中 7例均表达Vβ3.对 6例GVHD患者的基因扫描显示 ,均存在克隆性T细胞生长 .结论 移植后病人外周血淋巴细胞TCRVβ基因片段部分受抑制 ,部分基因片段呈选择性扩增 .GVHD患者有Vβ3和Vβ8基因的优势表达 。

(Department of Hematology, Guangdong Provincial People's Hospital Guangzhou 510000 China) Objective To evaluate to investigate restricted expansion of TCR Vβ gene repertoire and the reconstitution of T cell receptor repertoire following allogeneic hematopoietic stem cell transplantation. Methods Ten patients(9 males, 1 female)with 6 CML-CP and 4 AML-CR1 who underwent HLA-matching sibling or unrelated BMT and/or PBSCT .The peripheral mononuclear cells were obtained by Ficoll-Hypaque density gradient centrifugation from patients before and after treatment of MMF(1.0-2.0g/d). Results Only 2~18Vβgenes were found in samples from these ten patients within one year.TCR repertoire complexity was abnormal in all patients, parts of the genes were expansion and part of them were suppressed. Samples from 9 patients with GVHD show Vβ3 in 7 cases , Vβ8 and Vβ23 in 6 patients. Conclusions We found that T cells bearing restricted Vβ segments selectively expanded in the peripheral blood of cGVHD after transplantation. Only 5~22Vβ subfamily T cells were found in samples from these patients whose transplantation were more than one year, although the number of CD 3 + T cell have restored by flow cytometric analysis . Lastly, we found that TCRVβ3 gene maybe the target gene of MMF.
