摘要
目的 :研究重型脑损伤后血NO浓度变化与脑血管痉挛 (CVS)的关系。方法 :重型脑损伤病人 34例 ,入院时格拉斯哥评分GCS≤ 8分 ,入院后第 1~ 3天 ,4~ 7天 ,14天经颅多普勒超声 (TCD)测定大脑中动脉 (MCA)的血流速度及抽血分离血浆后测定NO的含量。结果 :19例 (5 6 % )发生脑血管痉挛 ;其中轻度 8例 ,中度 6例 ,重度 5例。痉挛组 (CVS)与非痉挛组 (non CVS)血NO浓度虽然在伤后均高于正常组 ,但两者存在明显差异 ,CVS组NO浓度升高程度与CVS的严重程度相符。结论 :重型脑损伤患者动态检测血NO含量变化 ,有助于判断伤后脑血管痉挛程度 ,对治疗具有指导意义。
s Objective:To elucidate the nitric oxide concentration changes and their relationship with posttraumatic cerebral vasospasm(CVS).Methods:Thirty four patients suffering from severe head injuries(GCS less than 8) were included into our study.Blood flow velocities were determined serially by TCD in MCA in day 1~3,4~7,14.meanwhile,concentration of plasma NO were measured serially in day 1~3,4~7,14,after severe head injury,and compared with those of normal volunteers.Results:CVS was found in 19 patients,the levels of NO in patients with and without CVS were significantly different.Conclusion:Detecting blood concentration of NO in patients with severe head injury is helpful to evaluate the extent of CVS,which is of instructive meaning to the following treatment.
