龈沟液弹性蛋白酶及其抑制因子水平与牙周炎相关性的研究 被引量:1

摘要 目的 :观察牙周病进展不同阶段龈沟液 (GCF)中弹性蛋白酶 (EA)和α1 抗胰蛋白酶 (α1 AT)水平的变化 ,探讨它们在牙周炎发病机制中的作用以及在牙周炎诊断中的价值。方法 :分别采用底物反应法和ELISA法检测GCF中EA和α1 AT水平。结果 :GCF EA活性及EA活性与α1 AT总量的比值 (EA/α1-AT)与各临床指标正相关。不同牙周状态下EA活性以及EA/α1 AT间的差异有极显著性 ,随牙周组织炎症破坏的加重 ,EA活性及EA/α1 AT逐渐升高。结论 :GCF EA活性、EA/α1 AT比值可反映取样牙位的临床状态并与各牙周临床指标密切相关 。 Objective:To study the correlation of elastase(EA) and α 1-antitrypsin (α 1-AT) level in gingival crevicular fluid(GCF) of different periodontal status and their roles in periodontal inflammatary pathogenesis. Methods: 62 volunteers aged 45~51 years old were enroled.Their periodental status were examined and grouped into healthy periodontium (H),8 cases,marginal gingivitis (MG),12 cases,mild chronic periodontitis (MCP),20 cases and advanced chronic periodontitis (ACP),22 cases.EA in GCF were measured with a chromogenic low molecular weight substrate reaction and the α 1-AT with ELISA. Results: Significantly positive correlation was found between GCF-EA activity and clinical periodontal parameters (P<0.05 or P<0.01), so was the ratio of EA to α 1-AT (EA/α 1-AT) (P< 0.05 or P<0.01). GCF-EA and EA/α 1-AT in group H were lower than those in other groups (P<0.01) and were heigher in group ACP than in group MCP or MG (P<0.05). Conclusion: The increase of EA activity and EA/α 1-AT in GCF may reflect the presence of inflammatory destructive pathogenesis of periodontal tissues.
