摘要 目的:探讨黏附分子CD44V6和原位癌基因Bcl-2在宫颈癌中的表达及其发生发展过程中的意义。方法:应用免疫组化SP法检测153例宫颈癌、正常宫颈上皮组织和宫颈不典型增生中CD44V6和Bcl-2的表达,并分析其与临床病理间关系。结果:①CD44V6表达在宫颈癌中最高,其次为宫颈不典型增生,在正常宫颈组织最低,差异有显著性(P<0.05);组织学分级越高,CD44V6表达率越高,并与组织学分级显著相关(P<0.05);淋巴结转移组明显高于无淋巴结转移组(P<0.05);②Bcl-2表达在宫颈癌中最高,其次宫颈不典型增生,在正常宫颈组织不表达,差异有显著性(P<0.05);与临床分期、组织学分级呈负相关,且差异均有显著性(P<0.05);淋巴结转移组低于无淋巴结转移组,但差异无显著性(P>0.05);③CD44V6和Bcl-2在宫颈癌中表达不相关(P>0.05)。结论:CD44V6、Bcl-2表达与宫颈癌的发生、发展及转移密切相关,可作为预测宫颈癌预后的辅助指标。 Objective: To investigate the expression of CD44V6 and Bcl-2 in Cervical carcinoma and its clinical significance during advance of this kind of cancer. Methods: One hundred and fifty-three cases with cervical carcinoma, normal cervical epithelium and cervical dysplasia were investigated on the expression of CD44V6 and Bcl-2 with the method of immunohistochemistry SP. Results:(1) The positive rate of CD44V6 was 66. 67% in cervical carcinoma and 44. 44% in cervical dysplasia and 8. 33% in the normal cervix epithelium ( P < 0. 05) . The higher pathological grade was, the higher expression was. There was significant difference between expressive rate of CD44v6 and pathological grade ( P < 0. 05), but no difference between clinical stage and expressive rate ( P > 0. 05). CD44v6 staining was remarkably higher in the group with metastatic lymph nodes (88. 89% ) than in the group without metastasis (41. 38% , P > 0. 05). (2) The positive rate of Bcl-2 was 54. 47% in cervical carcinoma and 33. 37% in cervical dysplasia and there was no expression in the normal cervix epithelium ( P < 0. 05). Bcl-2 staining in cervical cancer was negatively correlated with pathological grades and clinical stages, and there was significant difference in them (plower in the cases with metastatic lymph nodes than in the cases without metastasis , 58. 62% ). But there was no significant difference between the two groups ( P > 0. 05). (3) There was no difference between expression of CD44v6 and Bcl-2 ( P > 0. 05). Conclusion: the expression of CD44V6 and Bcl-2 may be linked with the pathogenesis, advance and metastasis of cervical carcinoma. Determination of CD44V6 and Bcl-2 may be useful for predicting the prognosis of cervical carcinoma.
