摘要
目的:探讨彩色多普勒超声(CDFI)在下肢深静脉血栓形成中的应用价值。方法:通过彩色多普勒对138例急、慢性下肢深静脉血栓形成病人的诊断和疗效的观察进行分析和探讨。结果:急性血栓117例,髂-股静脉65例,占55.6%;股静脉26例,占22.2%;腘静脉15例,占12.8%;髂-股-腘静脉11例占9.4%。超声表现为增宽的血管内低回声,管壁加压不变扁,CDFI管腔内无血流或血流充盈缺损。本组患者抗凝、溶拴治疗后大部分都有不同程度的改善。慢性血栓21例,血管粗细不均,内为稍强回声,8例CDFI内见“轨道征”血流,13例侧支循环形成。结论:彩色多普勒为下肢深静脉血栓形成提供了早期、无创和具有高敏感性、特异性的检查方法,对指导临床用药并观察疗效有重要价值。
Objective: To evaluate application of Color Dopple Flow Imaging (CDFI) in diagnosis of deep venous thrombosis of lower extremites. Methods: CDFI was applied to diagnose 138 patients with deep lower vena thrombosis and evaluate its therapeutic efficacy. Results: There were 117 cases with acute venous thrombosis, of whom 65 cases were afflicted with iliac-femoral venous thrombosis (55. 6% ), 26 cases with femoral venous thrombosis (22. 2% ), 15 cases with popliteal venous thrombosis (12. 8% ), and 11 cases with iliac-femoral-popliteal venous thrombosis(9. 4% ) . After treatment with anticoagulant and thrombolytic agents, various degrees of re-canalization were displayed in most of them. Somewhat little higher echoes and unequal diameter in the vein exhibited in 21 cases with chronic venous thrombosis, of whom the color blood flow of railway sign was shown in 8 cases and the compensatory circulation was formed in 13 cases. Conclusion; CDFI, as a highly sensitive, specific and non-invasive procedure used at the early stage to diagnose deep lower vena thrombosis, has great value to clinical practice and evaluation of therapeutic efficacy.
