摘要
目的 探讨冠状动脉搭桥合并室壁瘤切除手术中体外循环 (CPB)的方法 ,提高手术成功率。方法 在 9例冠状动脉搭桥合并室壁瘤切除术中 ,应用德国生产的RFD2 0 - 973型磁悬浮离心泵 ,作为CPB供血泵。采用鼻咽温 30~ 34℃ ,高流量 (2 .4~ 3.2 )L/ (min·m2 )灌注。平均动脉压维持在 5 0~ 80mmHg。CPB时间 80~ 12 0min(平均 110min) ,心肌血运阻断时间 4 2~ 90min(平均 76min)。结果 全组患者均自动复跳 ,无一例出现血红蛋白尿 ,无CPB并发症及手术死亡。术后患者心绞痛消失。结论 离心泵具有血液有形成份破坏小 ,安全性高 ,操作简单 。
OBJECTIVE To report 9 cases of Coronary Artery Bypass and Aneurysmectomy, and using centrifugel pump. METHODS German RFD20-973 Centrifugal Pump was used in cardiopulmonary bypass(CPB). Perfusion was carried out in the state of mild hypothermia(30~34℃) and high blood flow(2.4~3.2 L/(min·㎡).The average artery pressure ranged from 50 to 80 mmHg. The duration of CPB ranged from 80 to 120 minutes(mean 110), and the time of aortic cross clamping was 42 to 90 minutes(mean 76). RESULTS Among all the patients, spontaneous resuccitation with no hemoglobinuria, or other complications. All patients survived the surgery and no longer had angina.CONCLUSION Centrifugel pump is an ideal blood pump for coronary artery bypass and aneurysmectomy.
