摘要
目的 探讨伽玛刀 (γ 刀 )立体定向放射外科治疗原发性三叉神经痛的方法和疗效。方法 对 38例原发性三叉神经痛患者采用MRI扫描定位、Gamma TPS系统定位和治疗规划 ,OUR XGDγ 刀行患侧三叉神经痛立体定向放射外科治疗。治疗靶点位于三叉神经根近脑干侧 ,其中 35例采用双 4mm准直器 ,另外 3例采用单 4mm准直器 ,中心剂量为 6 0~ 80Gy。 结果 经过 3~ 36个月 ,平均 2 6个月临床随访 ,治愈 2 2例 (5 7.9% ) ,显效 9例 (2 3.7% ) ,有效 5例 (13.2 % ) ,总有效率达到94 .8% ,无任何并发症。结论 γ 刀治疗原发性三叉神经痛是安全有效的方法。
Objective To investigate the methods and effectiveness of the treatment of trigeminal neuralgia by Gamma knife radiosurgery.Methods Target localization and treatment plan were made by MRI study and Gamma TPS plan system. OUR XGD Gamma knife was employed for stereotactic radiosurgery treatment of trigeminal neuralgia. Double 4 mm collimator was used in 35 cases, single 4 mm collimator was used in 3 cases . The central dose of 60~80 Gy was irradiated to the proximal trigeminal nerve root from the pons.Results Follow up from 3 to 36 months(mean 26 months), 22 patients(57.9%) completely recovered, 9( 23.7 %) had obvious improvement, 5(13.2%) had partial result, the total effective rate was 94.8%. No complications were found during follow up.Conclusion Gamma knife radiosurgery is an effective and safe therapeutic method for trigeminal neuralgia.
