摘要
①目的 探讨银屑病的发病与皮损中角质形成细胞 (角朊细胞 )凋亡及T细胞亚群的关系。②方法采用末端脱氧核苷酰转移酶 (TdT)介导的d UTP生物素缺口末端标记 (TUNEL)技术、流式细胞技术 ,检测银屑病病人皮损中角朊细胞凋亡情况 ,采用免疫组织化学染色方法 (SABC)检测皮损组织中T淋巴细胞亚群 (CD4 + 、CD8+ 、CD2 5 + )的原位表达情况。③结果 银屑病病人皮损中角朊细胞发生凋亡的比例高于正常对照组 ,但病程的不同阶段角朊细胞凋亡指数 (AI)不同 ,以进行期为最高 (F =2 70 .0 6 ,q =3.12、2 .85 ,P <0 .0 1)。银屑病病人皮损组织部位表皮及真皮CD4 + 、CD8+ 、CD2 5 + T细胞的原位表达均显著高于对照组 ,进行期皮损组织表皮及真皮的CD4 + 、CD2 5 + T细胞的原位表达均显著高于静止期 ,而CD8+ T细胞表皮的原位表达明显低于静止期 ,差异均有显著性 (F =10 4 .2 0~ 10 97.0 0 ,q =3.0 6~ 6 .2 1,P <0 .0 1)。④结论 角朊细胞凋亡可能是银屑病发病过程中的重要事件 ,T细胞亚群的改变在银屑病的皮损形成过程中有重要作用 。
Objective To study the relationship of keratinocyte apoptosis and T-lymphocyte subsets in psoriatic epidermis. Methods Apoptosis of keratinocytes in psoriatic lesions and normal skin were measured by terminal deoxynucleotidyl transferase-mediation dUTP-biotin nick end labelling (TUNEL) and flow cytometer respectively. An immuno-histochemical technique was employed to study the in-situ expression of infiltrating T lymphocytes(CD4 +,CD8 +,CD25 +) in the lesioned psoriatic skin. Results The higher frequency of apoptotic keratinocytes was observed in progressive stage of psoriatic lesion, and the frequency reduced in static stage psoriatic lesion and normal skin (F=270.06, q=3.12,2.85, P<0.01). There was a significant increase of expression of infiltrating T-lymphocytes (CD4 +,CD8 +,CD25 +) in the lesioned skin than in normal one. The expression of CD4 +,CD25 + T cells in progressive stage lesioned skin was higher while the expression of CD8 + T cells was lower than in static stage lesion (F=104.20-1 097.00, q=3.06-6.21,P<0.01 ) . Conclusion There may be a close relationship between the development of psoriatic lesion and apoptosis of keratinocytes. Immunology indicator in psoriatic epidermis may play an important role in keratinocytes apoptosis.
