摘要 背景与目的:近年来有报道,应用逆转录-多聚酶链反应(reversetranscription-polymerasechainreaction,RT-PCR)技术检测有关肿瘤细胞标志物的mRNA表达来判断淋巴结转移情况,可以获得较高的检测灵敏度。本研究的目的在于探讨RT-PCR检测癌细胞淋巴结转移的临床意义。方法:应用RT-PCR技术检测患者腋窝淋巴结中MUC1和keratin19的mRNA表达,结合HE染色结果判断有无发生腋窝淋巴结转移。HE染色阴性患者随访3年。结果:86例肿瘤组织中可检测出MUC1和keratin19的表达,而正常淋巴结中未发现。在37例HE染色淋巴结阴性的病例中,有11例(29.7%)的keratin19检测结果阳性,这部分病例的3年复发率为45.5%,而keratin19表达阴性的病例其3年复发率仅为12.5%(P<0.01)。结论:RT-PCR检测出的MUC1和keratin19在淋巴结中的表达可作为乳腺癌转移的参考标志,有助于对乳腺癌预后的判断。 BACKGROUND &OBJECTIVE:Recently,it has been reported that reverse transcription polymer ase chain reaction(RT-PCR)for the detection of related tumor marker g enes expressi on in lymph nodes can be used to evaluate tumor metastasis and results in hig her sensitivity.The objective of thi s study was to investig ate the clinical sig nificance of RT-PCR for detecting axilla ry lymph node metastases of breast cancer.METHODS :RT-PCR was used to examine the mRNA of MUC1and keratin19in axillar y lymph nodes.The lymph node metastases was assessed referring t o the results of both RT-PCR and hematoxylin-eosin(HE)staining .All 37patients with neg at ive HE staining had been followed up for at least three ye ars.RESULTS :Both MUC1and keratin19expression were detected in all 86br east cancer specimens ,while it was undetectable in all lymph nodes from patients with benig n diseases.Amon g 37patients with neg ative HE staining i n lymph nodes ,11were positive for ke ratin19expression(29.7%),whose 3-year recurrence rate was 45.5%(5/11),while the patients with neg ative keratin19expression was just 12.5%(2/26,P<0.01).CONCLUSION:The expression of MUC1and keratin19in lymph nodes detected by RT-PCR can be reg arded as a sig n of breast cancer micrometastasis ,which can be used t o evaluate prog nosis in patients as w ell as to choose the postoperative treatme nt protocols.
