摘要
目的通过对室间孔区域的显微解剖,结合临床经验,设计新型的手术入路,以提高室间孔区域肿瘤的手术效果。方法选择侧脑室室间孔区域肿瘤16例,行改良经额皮质侧脑室前入路,作为研究组(第1组);选择同期常规手术方法治疗的室间孔区域肿瘤25例,作为对照组(第2组);将两组资料进行比较。结果现有的室间孔区域肿瘤的手术方法,临床效果仍不满意。改良经额皮质侧脑室前入路切除室间孔区域肿瘤,手术全切率为81.2%,术后恢复良好。结论改良经额皮质侧脑室前入路切除室间孔区域肿瘤的手术视野扩大,易于操作,皮质功能保护肯定,手术效果较为满意。
Objective The operative effect was no t satisfactory in foramen of Mon ro region.Based on the microanatomical finding, we designed a new operation approach,named modi fied anterior transcortical ap proach,to resect tumours in foramen of Monro region.Methods There were two o peration groups of tumours in foramen of Monro region.16cases in Group-One( Research Group)were taken modified anterior transcortical approach.And25case s in Group-Two (Contrast Group)were taken routine operation approaches.Karno fsky performance scale(KPS)was adopted as follow-up standard.Compare the ope ration effect were compared between Group-One and Group-Two.Results There w ere two types of approaches,which were anterior transcallosal approach and ant erior transcortical ap proach.The anterior transcallosal approach was frequentl y hindered by the superior cere bral veins of frontal lobe.And the anterior tra nscortical approach was difficult to perform because of limited operation sight .The operation effect of modified anterior transcortical approach to foramen of Monro region was quite satisfied,which resulted in higher total tumours' resec tion ratio (81.2%).Conclusion The modified anterior transcortical approa ch was better than routine operation approach to foramen of Monro region.The op eration sight was field enlarged,and the protection in cortical function area was much better.
