侧脑室肿瘤手术入路的选择 被引量：6

摘要 侧脑室肿瘤作为脑室肿瘤的一部分也分为原发性和继发性两种，其中起源于侧脑室各结构（包括脉络丛、脉络体和室管膜）的肿瘤称为原发性侧脑室肿瘤，原发于侧脑室壁附近脑组织而肿瘤大部分（超过2，3）突人侧脑室内者称为继发性侧脑室肿瘤。虽然侧脑室肿瘤部位深，但大部分可经外科手术成功切除，但是也存在着一定的并发症和1％～3％的死亡率。因此术前必须充分考虑病人已有的神经功能障碍和手术目的，同时根据肿瘤的部位、大小、生长方式、病理性质以及肿瘤的血供和脑室大小，选择最佳的手术入路。理想的手术人路应既能充分暴露肿瘤又能最大限度地减少对脑组织及重要结构的损害。术前对肿瘤的血供以及肿瘤分块切除的可能性应有充分评估。这有利于术中早期判断并处理肿瘤的血供，避免对脑组织过度牵拉，减少对脑组织尤其是重要功能区的损伤。手术切除侧脑室肿瘤不可避免地会造成部分脑组织的损伤．但应避免损伤一些重要的结构如中央区、语言中枢、穹隆（至少保留一侧）、大脑内静脉和Galen静脉等。
