摘要
背景与目的:原发性肝癌(hepatocellularcarcinoma,HCC)是全世界范围内常见的肿瘤之一,HCC的诊断定性一直是比较困难的问题。本研究主要探讨超顺磁性氧化铁(superparamagneticironoxide,SPIO)增强MRI在HCC诊断中的作用,及其与HCC组织学分级的相关性。方法:对30例怀疑肝脏病变患者行MRI平扫和SPIO增强扫描,分别测量增强扫描前后肿瘤区和非肿瘤区肝脏信号的强度,计算病灶的SPIO强度比。将手术所得病灶标本行枯否细胞免疫组化检查,计数枯否细胞,并计算枯否细胞计数比。结果:增生结节枯否细胞计数比(1.05±0.12)高于HCC病灶(P<0.05),高分化HCC枯否细胞计数比(0.81±0.12)高于低分化HCC(0.15±0.14)和中等分化HCC(0.38±0.21)(P<0.01)。病灶枯否细胞计数比与SPIO强度比呈负相关(r=0.64,P<0.01)。结论:SPIO增强可反映原发性肝癌及增生结节内枯否细胞的数目,预测原发性肝癌的组织学分级,并有助于鉴别原发性肝癌与增生结节。
BACKGROUND &OBJECTIVE:Hepatocellular carcinoma (HCC)is one of the most common tumors in the world.The diag nosis of HCC is very difficult.This study was desig ned t o evaluate the role of SPIO(superparamag netic iron oxide )-enhanced mag netic resonance imag i ng(MRI )in diag nosing hepatocellular carcinoma and its co rrelation with histolog ical g radin g .METHODS :Conventional plain and Feridex-enhanced MRI scanning was performed in 30patients with suspec ted hepatic lesions.The sig nal inte nsity and Kupffer-cell-count of lesions and n ontumorous area was observed.SPIO i ntensity ratio and the ratio of Kupffer-cell-count were analyzed.RESULTS :Ratio of Kupffer-cell-count in well-differ entiated HCC(1.05±0.12)was hig her than those in moderately and poorly-different iated HCC(0.38±0.21and 0.15±0.14,respectively)(P<0.01).The ratio of the intensity of tumor l esions to that of nontumor area on SPIO-enhanced MRI i nversely related to Kupffer-cell-c ount ratio in hepatic lesions (r=0.064,P<0.01).CONCLUSION:SPIO-enhanced MRI may reflect Kupffer-cell-count in HCC and dysplastic nodules and is u seful for estimation of histolog ical g rad ing in HCC.
