TAT和GMP-140在急性心肌梗塞患者溶栓治疗中的应用价值

Value of plasma thrombin-antithrombin complexity and alpha granule membrane protein in patients with acute myocardial infarction treated by thrombolytic therapy

摘要 目的 探讨急性心肌梗塞溶栓治疗中凝血酶 -抗凝血酶复合物 (TAT)和血小板α 颗粒膜蛋白 (GMP 14 0 )动态变化及临床应用价值。方法 以健康人为正常对照组 (n =4 8) ,随机选择 6h内急性心肌梗塞患者 4 8例 ,用 15 0万U尿激酶进行溶栓治疗 ,并按临床再通标准分为再通组 (n =30 ) ,未通组 (n =18) ,分别在溶栓前、溶栓后 2h、4h、2 4h、4 8h采用酶联免疫吸附双抗体夹心法测定血浆TAT和GMP 14 0水平 ,观察其动态变化。结果 患者组治疗前TAT水平 (14 .5 6± 1.0 4 ) μg L、GMP 14 0水平 (12 .5 0± 1.0 5 )ng ml,较正常对照组 (2 .15± 0 .89) μg L、(8.39± 1.33)ng ml显著升高 (P <0 .0 5 ) ,治疗后 2h再通组GMP 14 0水平达到峰值 (2 2 .17± 2 .0 5 )ng ml,4h后TAT到达峰值 (46 .6 7±5 .79) μg L(P <0 .0 5 )后逐渐下降 ,持续至少 3天 ,但仍高于正常对照组。未通组二项指标持续缓慢上升 ,4 8h达到峰值。结论 急性心肌梗塞溶栓治疗前后TAT、GMP - 14 Objective To explore the value and dynamic changes of thrombin antithrombin complexity(TAT) and alpha granule membrane protein(GMP 140) in patients with acute myocardia infarction(AMI) before and after thrombolytic therapy.Methods Thrombolytic group was devided into reperfusion group and nonreperfusion group. The levels of plasma TAT and GMP 140 were measured in 48 patients with AMI by ELISA before thrombolytic therapy and at 2h,4h,24h,48h after thrombolytic therapy, compared with control group( n =48).Results The levels of TAT(14.56±1.04)μg/L and GMP 140(12.50±1.05)ng/ml for patient group were significantly elevated than those in control group ( P <0.05), In patients with reperfusion group GMP 140 peaked at 2h after therapy at(22.17±2.05)ng/ml,TAT peaked at 4h after thrombolytic therapy at(46.67±5.79)μg/L, then decreased slowly,and sustained for at least three days,but were still higher than those in control group. In patients nonreperfusion group,the levels of two parameters increased slowly,peaked at 48h.Conclusion Combined measurements of TAT and GMP 140 play an important role in judging the results of thrombolytic therapy in patients with AMI.
