摘要
目的探讨筋膜内子宫切除术对子宫肌瘤患者性功能、婚姻和生活质量的影响,寻找手术治疗的最佳方案。方法采用国际女性性功能评估量表,诺丁汉健康调查表和Olson婚姻质量调查表测试107例治疗组(筋膜内子宫切除术)和65例对照组(普通传统子宫切除术)术前和术后3个月的各项得分。结果两组患者的术前各种量表评估都无明显区别(P均>0.05),但术后3个月所有量表评估得分均显著优于手术前(t=2.054～2.470,2.147～2.518,P均<0.05),治疗组术后3个月的国际女性性功能量表评估中,除性行为异常症状评分外,余下各项均明显优于对照组(t=2.199～2.235,P均<0.05),而在Olson婚姻质量调查中治疗组术后3个月的婚姻满意度异性生活评分分别为39±7和46±8,明显高于对照组35±6和40±8(t=2.776,2.458,P均<0.05),两组术后3个月的生活质量评分接近(P均>0.05)。结论筋膜内子宫切除术对子宫肌瘤患者性功能和婚姻质量有正性影响。
Aim To explore the effect of intrafascial hysterectomy on sexual function, quality of life and quality of marital life in women with hysteromyoma, seeking the idealest programme of operation.Methods The Brief Index of Sexual Function for Women(BISF W),Nottingham Health Poofity(NHP)and Olson's ENRICH Questionary(OEQ) were tested 3months before and after treatment among 107 patients treated with intrafascial hysterectomy as treatment group and 52 patients treated with general hysterectomy as control group.Results All the scores before operation in two groups were not significantly different (P >0.05).But 3 months after all the scores in two groups(t=2.054-2.470, t=2.147-2.518) were more significantly different than before (P < 0.05); the majority scores of BISF W and OEQ in treatment group were significantly higher than those in control group except the scores of abnormal symptom(t=2.199-2.235, P < 0.05); the Olson scores of the treatment group(39±7 and 46±8) were significantly higher than those of the control group(35±6, 40±8)(t=2.776, 2.458, P< 0.05).Conclusion Intrafascial hysterectomy is positively correlated with the sexual function and quality of marital life in patients with hysteromyoma .
