摘要
目的 研究2型糖尿病(T2DM)患者性激素水平与胰岛素(INS)、胰岛素抵抗(IR)、胰岛素敏感性的关系。方法 对63例T2DM患者(男35例,女28例)以及正常对照组45例(男24例,女21例),用放免法测定雌二醇(E_2)和睾酮(T),用酶联免疫测定法测定INS,计算IR和胰岛素敏感指数(ISI)并进行比较。结果 T2DM患者存在性激素紊乱。女性DM组E_2与INS、IR呈负相关(分别是r=-0 38,P<0.05,r=-0.42,P<0.05),与ISI呈正相关(r=0.53,P<0.01);T与INS、IR呈正相关(分别是r=0.48,P<0.01;r=0.74,P<0.01);与ISI呈负相关(r=-0.77,P<0.01)。男性DM组,E_2与INS呈正相关(r=0.36,P<0.05);T与INS、IR呈负相关(分别是r=-0.37,P<0.05;r=-0.47,P<O.01),与ISI呈正相关,(r=0.52,P<0.01)。结论 T2DM患者存在性激素紊乱,且与INS、IR、ISI相关,T是男性DM的保护激素,E_2是女性DM的保护激素。
Objective To study the relationship between the levels of sex hormones and the insulin(INS) concentrations, IR as well as insulin sensitivity(ISI) in type 2 DM (T2 DM)patients. Methods 63 T2 DM patients (35 men and 28 women) and 25 healthy objects (24 men and 21 women)were observed. The levels of estradiol(E2) and testosterone (T) were measured by RIA in 63 cases with T2 DM. The INS concentrations were measured by ELISA and IR and ISI were calculated. Results The patients with T2 DM had sex hormone disturbances. In the women with T2 DM, E2 was negatively correlated with INS or IR (r1 = -0. 38, P<0. 05; r2 = -0. 42, P<0. 05) and positively correlated with ISI (r=0. 53, P<0. 01); the T was positively correlated with INS and IR (r1 = 0. 48, P<0. 01; r2 = 0. 74, P<0. 01). In the men with T2 DM, the positive correlation was found between E2 and INS (r=0. 36, F<0. 05), E2 was not correlated with IR and ISI; the T was negatively correlated with INS and IR (r1 = -0. 37, P<0. 05; r2 = - 0. 47, F<0. 01) and the positive correlation between T and ISI (r=0. 52, F<0. 01) was found. Conclusions The patients with T2 DM have disturbance of sex hormones which is related with INS, IR and ISI. In the patients with T2 DM, the T is the protective hormone in the men, and the E2 in the women.
