乳腺癌术前化疗后肿瘤组织中VEGF、Flk-1的表达和血管生成

摘要 目的 探讨乳腺癌组织中血管内皮生长因子 (VEGF)和受体 (Flk 1 )的表达与微血管密度 (MVD)的关系 ,了解术前化疗对乳腺癌VEGF、Flk 1和MVD的影响。方法 对 44例术前化疗的乳腺癌和 32例未术前化疗的乳腺癌标本 ,进行VEGF、Flk 1和CD34免疫组化标记 ,并进行微血管计数 (MVC)。结果 术前化疗组和未化疗组VEGF阳性率分别为 45 5 % (2 0 / 4 4 )和 53 1 % (1 7/ 32 ) ;Flk 1的阳性率分别为 38 6 % (1 7/ 4 4 )和 56 3 % (1 8/ 32 ) ;两组MVC分别为 2 4 5± 1 2 5 ,2 6 1± 1 1 3 ,经统计学处理均差异不显著 (P >0 0 5)。化疗组VEGF及Flk 1、未化疗组Flk 1的表达与否 ,其MVC均无显著性差异 (P >0 0 5) ;未化疗组VEGF阳性和阴性病例 ,MVC分别为 30 4± 1 1 6、2 1 6± 1 3 3 ,两者差异显著 (P <0 0 5)。结论 常规术前化疗并未显示出明显的血管生成抑制作用 ,也未显示出VEGF和Flk Objective To investigate the effects of the neoadjuvant o n angiogenesis and expression of VEGF and Flk-1 in invasive breast cancers Methods Total 44 cases with neoadjuvant versus 32 cases with pr imary operation were studied Immunohistochemical staining was performed with C D34, VEGF and Flk-1 monoclonal antibodies, and the scoring of microvessel (MVC) was performed using the manual of 'hot-spot' procedure Results VEGF and Flk-1 were detected in 20 and 17 out of 44 cases with neoadjuvant, and in 17 and 18 out of 32 cases with primary operation The mean scores of MVC were 24 5±12 5 and 26 1±11 3 respectively There were no statistical sig nificance between the neoadjuvant group and the primary operation group in the e xpression of VEGF, Flk-1 or in score of MVC There was no significant differen ce in score of MVC between the positive cases and negative cases of VEGF and Flk -1 in the neoadjuvant group The score of MVC in cases with VEGF positivity w as significant higher than that of VEGF negativity in primary operation group ( P<0 05) Conclusion Neither inhibitory effect of angiogenesis nor chang ed expression of VEGF and Flk-1 is observed after primary chemotherapy
