摘要
目的 了解肺炎克雷伯菌 (Kpn)所产各种 β 内酰胺酶 ( β lase)分布情况。方法 采用多底物改良相邻纸片法检测 10 5株呼吸道、非呼吸道Kpn。结果 10 5株Kpn中产β lase菌株 49株 ,总β lase检出率为 46 .7%,其中产青霉素酶、头孢菌素酶各 2株 ,广谱酶 11株 ( 10 .5 %) ,产ESBLs34株 ( 32 .4%) ,未检出培南酶 ;34株产ESBLs菌株中 ,CAZ、ATM、CTX检出率分别 82 .4%,85 .9%,94.0 %;6 9株呼吸道的Kpn产 β lase 39株 ( 5 6 .5 %) ,其中青霉素酶 1株、头孢菌素酶 2株 ,广谱酶 10株 ( 14.5 %) ,ESBLs2 6株 ( 37.7%) ,而 36株非呼吸道Kpn产β lase 10株( 2 7.8%) ,其中青霉素酶、广谱酶各 1株 ,ESBLs 8株 ( 2 2 .2 %)。结论 本组Kpn菌产 β lase以广谱酶、ESBLs为主 ,未检出培南酶 ;本组Kpn菌所产ESBLs的检出率以CTX最高 ;呼吸道的Kpn的产酶率、产ESBLs率均高于非呼吸道的Kpn。
OBJECTIVE To study the classification and distribution of the β-lactamases produced by Klebsiella pneumoniae. METHODS To detect the β-lactamases produced by K. pneumoniae with a multi-disk synergy test. RESULTS Forty nine strains of 105 strains produce the β-lactamases, total positive rate is 46.7%,each two strains produce penicillinases or cephalosporinases,11 strains produce broad-spectrum β-lactamases(BSBLs,10.5%), 34 strains produce extended-spectrum β-lactamases(ESBLs,32.4%), all do not produce carbapenem-hydrolyzing β-lactamases(CHBLs); in 34 strains producing ESBLs, the sensitivities to the ceftazidime,aztreonam and cefotaxime are 82.4%, 85.9% and 94.0%,respectively; there are 39 strains producing the β-lactamases(56 5%)in 69 strains from respiratory tract, one strain producing penicillinases,two producing cephalosporinases,10 producing BSBLs (14.5%), 26 producing ESBLs(37.8%); 10 producing the β-lactamases(27.8%)in 36 strains from non-respiratory tract, one producing penicillinases,1 produces BSBLs, 8 producing ESBLs(22.2%). CONCLUSIONS The types of β-lactamases produced by the 105 strains are mainly BSBLs and ESBLs, all do not produce CHBLs; in the sensitivities for detecting ESBLs produced by the 105 strains, cefotaxime is the highest; The rates of β-lactamases, ESBLs produced by K. pneumoniae isolates from respiratory tract are higher than from non-respiratory tract.
