摘要
目的 探讨颈动脉内膜 中膜厚度 (IMT)与 2型糖尿病性脑梗死之间的关系。方法 应用B超技术对糖尿病性脑梗死患者的颈动脉IMT进行测量 ,并分析颈动脉IMT与脑血管病危险因素间的关系。结果糖尿病性脑梗死组的血纤维蛋白原 (FIB)、总胆固醇 (TC)和低密度脂蛋白 (LDL)水平显著大于对照组 (P <0 0 5 ) ;而高密度脂蛋白 (HDL)、ApoA1和ApoA1/ApoB的比值水平明显低于其他 3组 (P <0 .0 5 )。糖尿病性脑梗死患者的颈总动脉IMT、颈内动脉IMT与对照组和非糖尿病性脑梗死组相比有显著差异 (P <0 .0 5 )。Logistic回归分析提示颈总动脉IMT增厚与年龄、FIB、TC和HDL密切相关 (P <0 .0 5 ) ,与血糖和LDL水平无明显相关 (P >0 .0 5 )。结论 血糖本身可能不是导致颈动脉IMT增厚的主要因素 。
Objective To study the relationship between carotid intima media thicknes(IMT) and cerebral infarction in patients with type 2 diabetes.Methods The IMT of the carotid artery was measured by high resolution B mode ultrasound, the relationships between carotid IMT and risk factors of cerebrovascular disease were evaluated.Results The serum levels of fibrinogen, total cholesterol and low density lipoprotein cholesterol were significantly higher in diabetic subjects with cerebral infarction than those in control group ( P <0.05). However, The levels of high density lipoprotein cholesterol, apolipoprotein A 1(ApoA 1) and ratio of ApoA 1/apoB were significantly lower in diabetic subjects with cerebral infarction than in other groups ( P <0.05). IMTs both in the common carotid artery (CCA) and internal carotid artery (ICA) in diabetic subjects with cerebral infarction were significantly greater than control group and nondiabetic subjects with cerebral infarction( P <0.05). By Logistic regression analysis, we found increased CCA IMT was significantly associated with age, the levels of fibrinogen, total cholesterol and high density lipoprotein cholesterol, but not with the levels of blood glucose and low density lipoprotein cholesterol.Conclusion Blood glucose level may not be an important risk factor to which increase carotid IMT. Diabetes may develop increased carotid IMT and cerebrovascular disease by dyslipidemia and abnormal hemorheology.
