摘要 目的:颈动脉内-中膜厚度是反应颈动脉粥样硬化的最早期的指标。采用高分辨率超声能重复、清楚地检测颈动脉内-中膜厚度。临床研究表明颈动脉内-中膜厚度与缺血性心脑血管病密切相关。研究颈动脉内-中膜厚度的危险因素,是为了更好地认识动脉粥样硬化的发生发展,并达到预防脑卒中的目的。资料来源:应用计算机检索PubMed1991-01/2006-01颈动脉内-中膜厚度影响因素的相关文献,检索词为“imt,riskfactors”,限定文章语言种类为英语;计算机检索维普数据库2000-01/2005-12有关颈动脉内-中膜厚度影响因素的文章,检索词为“颈动脉内-中膜厚度,危险因素”,限定文章语言种类为中文。资料选择:对约650篇文献资料进行初审,纳入标准:①研究颈动脉粥样硬化、颈动脉内-中膜厚度的相关影响因素的文献。②随机、对照和盲法等论证推荐的文章。排除标准:Meta分析、综述文献及重复研究。资料提炼:共收集到100余篇关于颈动脉内-中膜厚度相关影响因素的文章,其中内容相似的,以近5年发表在较权威杂志者优先。对符合标准的48篇文献进行分析。资料综合:颈动脉内-中膜厚度是反应颈动脉粥样硬化的最早期的指标,与缺血性心脏病及脑卒中密切相关,有关其与心、脑血管疾病关系的文章较多,为了预防终点事件的发生,有关其影响因素方面的研究正日渐受到关注,尤其是基因方面的研究是当前的热点。结论:年龄、性别、吸烟、肥胖、基因改变、高血压、糖尿病、脂质异常、同型半胱胺酸及炎性反应可显著影响颈动脉内-中膜厚度。 OBJECTIVE: Intima-media thickness (IMT) of carotid artery can reflect the earliest atberosclerosis, which can be detected repeatedly and clearly by high-resolution uhrasonography. Clinical investigation demonstrated a close relationship between IMT and ischemic cardiac and cerebral vascular diseases. The aim of study on risk factors of IMT of carotid artery is to profoundly realize the occurring and progressing of carotid atherosclerosis, so as to prevent stroke. DATA SOURCES: PubMed was searched with computer for English articles about influencing factors of intima-media thickness of carotid artery published between January 1991 and January 2006 by using key words of "imt, risk factors". Meanwhile, a computer-based search of Vip Database was carded out for relevant Chinese articles published between January 2000 and December 2005. STUDY SELECTION: About 650 articles were selected firstly. Inclusive criteria: ①Literatures related to the influencing factors of IMT of carotid artery and artheroselerosis of caroid artery. ②Randomized, controlled and blind studies. Exclusive criteria: articles about Meta analysis, reviews and repetitive studies. DATA EXTRACTION: A total of 100 articles related to the influencing factors of IMT of carotid artery were collected, and those with similar contents and published in authoritative journals in the past 5 years were enrolled firstly. Forty-eight articles that in accordance with the inclusive criteria were analyzed. DATA SYNTHESIS: IMT of carotid artery could reflect the earliest atheroscleroais, and closely correlated with ischemic heart disease and stroke. There were much articles about the relationship between cardiac and cerebral vascular disease. To prevent the terminal events , researches on the risk factors of IMT was paid more and more attention to gradually, and researches on gene is a hot issue presently. CONCLUSION: Age, gender, smoking, obesity, genetic changes, hypertension, diabetes, dyslipidemia, Hcy, and inflammation can significantly affect the IMT of caroid artery.
