摘要
目的 运用阴道超声对经腹超声诊断欠明确的早期宫外孕做出诊断。方法 按照阴道超声操作常规 ,尽量侧动阴道探头显示扭曲的输卵管长轴与宫角、双侧卵巢的关系 ,以确定宫外孕的位置 ,探查子宫内、盆腔的情况 ,寻找宫外“胎囊状”结构。结果 2 5例中发现卵巢妊娠 1例 ,宫颈妊娠 1例 ,输卵管妊娠 2 3例 ,其中 6例手术证实 ,其余经氨钾喋呤治疗尿HCG转阴或诊刮病理示 :蜕膜样改变。结论 当尿HCG阳性 ,早期宫外孕包块小 ,盆腔积液不明显 ,经阴道超声为早期明确诊断的方法 ,避免病人在等侍中错过治疗的时机 。
Objective:Transvaginal ultrasound was performed in the diagnosis of earlier ectopic pregnancy which was unclear belly ultrasonography.Method:Extraterus pregnancy was situated in the relationship of twist tubal axis and ovaries by moving prober according to transvaginal ultrasound operation toutine.Structure of 'bag form of foetus'was found in probing of uterus and pelvic cavity.Result:Transvaginal ultrasound was performed in 25 cases ectopic pregnancy.Among them one with ovary pregnancy was found and another with uterus neck pregnancy.The remainder 23 cases presented tubal pregnancy,6 cases of which were verified on operation,and the others showing sloughed off the film and changed after the treatment of dilatation and curretage of urine HCG negative for ammonia potassium purine treatment.Conclusion:Transvaginal ultrasonography was clearly in diagnosis of the earlier ectopic pregnancy with smaller pregnant masses and free fluid in pelvic cavity to urine HCG positive.It ihas important clinical value to the obstetrical patients.
