摘要
目的分析输卵管妊娠患者期待治疗成功与失败的临床特点,总结影响期待治疗效果的因素。方法选择本院2013年1月-2014年8月诊治的80例输卵管妊娠患者为研究对象,均接受期待治疗,根据治疗结果将其分为失败组（需手术或药物治疗）、成功组（无需手术或药物治疗）,整理分析两组患者继往异位妊娠史、盆腔积液有无、血β-h CG水平等病例资料,对可能影响期待治疗效果的相关因素行Logistic回归分析。结果成功组异位妊娠史、囊性结构、血β-h CG≤635 U/L患者比例明显低于对照组（P〈0.05）。多因素Logistic回归分析提示血β-h CG、囊状结构及孕酮是影响输卵管妊娠期待治疗效果的危险因素（OR=1.50、2.32、1.23,P〈0.05）。结论输卵管妊娠患者期待治疗有一定的条件,治疗成功或失败的危险因素包括血β-h CG、囊性结构及孕酮。
Objective To analyze the clinical characteristics of success and failure of expectant treatment for patients with fallopian pregnancy,and to summarize the influencing factors for the curative effects of expectant treatment. Meth-ods 80 patients with fallopian pregnancy who were admitted and treated in our hospital from January 2013 to August 2014 were selected as research subjects.They all received expectant treatment.They were assigned to failure group （re-quired for surgery or medical treatment） and success group （not required for surgery or medical treatment） according to the treatment outcome,and materials of medical cases including previous history of ectopic pregnancy,wether the pa-tients had pelvic fluid,and levels of blood β-hCG between two groups was collected and analyzed respectivdy.Logistic regression analysis was carried out for potential relevant factors influencing the effects of expectant treatment. Results Proportion of patients with history of ectopic pregnancy,systic structure and blood β-hCG lower than 635 U/L in suc-cess group was significantly smaller than that in control group（P〈0.05）.In addition,multivariate Logistic regression anal-ysis indicated that blood β-hCG level,wether the patients had pelvic fluid and progesterone were risk factors influenc-ing the curative effects of expectant treatment for fallopian pregnancy （OR=1.50;2.32;1.23,P〈0.05）. Conclusion Expec-tant treatment for patients with fallopian pregnancy has certain conditions,and its risk factors for successful or failed treatment include bloodβ-hCG,cystic structure and progesterone.
