多平面经食道超声心动图在房间隔缺损封堵术中的应用 被引量:2

摘要 目的 探讨多平面经食道超声心动图 (MTEE)在引导Amplatzer封堵器堵闭继发孔房间隔缺损 (ASD)的应用价值。方法 术前经MTEE筛选的 2 3例继发孔ASD患者 ,在MTEE引导下以球囊测量ASD的最大伸展径 ,选择适当型号的Amplatzer封堵器经导管送至左房 ,封堵ASD。结果 术前MTEE所测ASD直径 (D1) 14 5~ 2 9mm[(18 7± 5 6)mm] ,最大伸展径 (D2 )为 18~ 3 2mm[(2 5 1± 7 3 )mm] ,D1与D2高度相关 (r =0 91,P <0 0 1)。选择的封堵器直径为 2 4~ 3 6mm[(2 6 4± 6 9)mm] ,2 3例患者均封堵成功 ,无并发症。术后 3月随访所有病例分流消失。结论 MTEE在筛选病例、选择封堵器型号。 Objective To evaluate the guidance of multi plane transesophageal echocardiography (MTEE) for transcatheter closure of secundum atrial septal defect (ASD) with Amplatzer occluder. Methods Transcatheter closure of ASD was performed on 23 MTEE selected patients with secundum ASD with the Amplatzer occlusion device under the guidance of MTEE. Results The preoperative sizes of ASD ranged from 14.5 to 29 mm [(18.7±5.6) mm] when measured with MTEE and from 18 to 32 mm [(25.1±7.3) mm] when with balloon occlusion catheter (stretched diameter). The sizes of the occluder were from 24 to 36 mm [(26.4±6.9) mm]. All patients underwent the procedure successfully, and had no complications. They all had complete closure of atrial shunting when evaluated in follow up of 3 months. Conclusion MTEE is essential in the evaluation of patients undergoing transcatheter closure of atrial septal defect.
