高度近视晶状体超声乳化吸出及后房型低或负度数人工晶状体植入术 被引量:4

摘要 目的 观察高度近视眼的晶状体超声乳化吸出联合低或负度数人工晶状体植入术的临床疗效及预后视力情况。方法  3 8例 47眼老年性、并发性、外伤性白内障及 4例6眼高度近视透明晶状体行巩膜隧道或透明角膜隧道切口超声乳化吸除 ,通过 3 .5 m m切口植入折叠式人工晶状体 13眼 ,通过 5 .5 m m切口植入硬性 PMMA人工晶状体 3 7眼 ,未植入人工晶状体 3眼。平均年龄 5 9.9岁 (19~ 83岁 ) ,眼轴长度 <3 0 m m 3 6眼 ,≥ 3 0 m m 17眼。人工晶状体度数≤ 0 D 6眼 ,0~ 10 D3 0眼 ,>10 D14眼。结果 视力 <0 .1者 4眼 ,0 .1~ 0 .4者 2 4眼 (45 .2 % ) ,≥ 0 .5者 2 2眼 (41.6% )。眼轴<3 0 m m组 ,术后最佳矫正视力≥ 0 .5者 17眼 ,占 47.2 % ,眼轴长度≥ 3 0 m m组 17眼 ,术后最佳矫正视力≥ 0 .5者 5眼 ,占 2 9.4% .术后无视网膜脱离、黄斑区水肿等严重并发症的发生。结论 高度近视眼晶状体超声乳化吸出及后房低或负度数人工晶状体植入术 ,具有术后视力恢复快、手术安全。 Objective To observe the therapeutic efficacy and visual acuity prognosis through lens extraction with phacoemulsification and implantation of low or negative degree intraocular lens(IOL) into posterior chamber for treatment of high myopia.Methods Phacoemulcification with implantation of 13 foldable IOL or 37 PMMA rigid IOL was performed on 47 eyes of 38 patients with senile, complicated or traumatic cataracts and 6 eyes of 4 patients with high myopia through a 3.2 mm small clear corneal tunnel incision or through a 5.5 mm scleral tunnel incision respectively. IOL were not implanted in 3 eyes. The average age of patients was 59.9 years. The axial length of 36 eyes was<30mm, 17 eyes was≥30mm. The diopter of IOL in 6 eyes was ≤0D, 30 eyes 0~10D, 14 eyes >10D.Results Postoperatively, the uncorrected or corrected visual acuity <0 1 was 4 eyes, 0 1 0.4 24 eyes (45.2%), ≥0.5 22 eyes (41.6%). ≥0.5 was 17 eyes (47.2%) in group that the axial length was≤30mm, 5 eyes (29.4%) ≥30mm . No retinal detachment and cystoid macular edema were observed in this series. Conclusion Phacoemulsification and low or negative posterior chamber implantation of IOL offers the advantage of rapid, better rehabilitation of visual acuity, safety and lower complication in high myopia.
