摘要
目的:探讨食管癌患者手术前后淋巴细胞及红细胞的免疫功能变化及相关性,为临床分期、治疗及预后评价提供理论依据。方法:采用ELISA法测定患者血清中sIL-2R、CD4、CD8的含量;采用郭峰法测定患者血清中RBC-C3bRR、RBC-ICR、TRR。结果:食管癌患者血清中sIL-2R和RBC-ICR水平明显增高(P<0.05);RBC-C3bRR和TRR水平明显降低(P<0.05);CD4水平降低,而CD8水平增高,CD4/CD8比值明显低于对照组(P<0.01)。术后14天sIL-2R较术前明显增高;RBC-ICR较术前明显降低;RBC-C3bRR和TRR均较术前明显增高,但仍低于对照组;CD4水平显著上升,CD8水平降低,CD4/CD8比值增高,但仍低于对照组;淋巴细胞和红细胞免疫功能随疾病分期的加重,呈渐进性降低。RBC-C3bRR与CD4/CD8比值之间呈正相关(r=0.68),与sIL-2R含量成负相关(r=-0.71)。结论:食管癌患者淋巴细胞和红细胞免疫功能低下。上述指标变化与临床分期、预后、转移及术后复发有关。另外,淋巴细胞与红细胞免疫测定结果有很好的相关性,由于红细胞免疫功能的测定方法简单、成本低廉、结果稳定,有望作为食管癌免疫功能检测的较好指标。
Objective:To study the changes and relationships between lymphocyte and erythrocyte immunologic function in patients with esophageal carcinoma,and find out some theory proofs for deciding clinical stage,therapy and prognosis.Methods:The serum sIL-2R,CD 4 and CD 8 level were detected by ELISA.The RBC-C 3 bRR,RBC-ICR and TRR level were detected by the method established by Guo Feng.Results:The sIL-2R and RBC-ICR level in patients were markedly increased(P<0.05);But the RBC-C 3 bRR and TRR level were markedly decreased(P<0.05);The CD 4 level was decreased and CD 8 level was increased,CD 4 /CD 8 was markedly decreased compared with those of contral group(P<0.01);In patients14days after operation,the level of sIL-2R was increased as compared with befor operation.The RBC-ICR level was decreased;the RBC-C 3 bRR,TRR and CD 4 /CD 8 level were increased,but still be lower than normal.There was positive correlation between RBC-C 3 bRR and CD 4 /CD 8 (r=0.68),and negative correlation with sIL-2R (r=-0.71).Conclusion:The lymphocyte and erythrocyte immunologicfunction were decreased in esophageal carcinoma patients.These changes had correlation with clinic stage,metastasis,recurrence and prognosis after operation.Between the lymphocyte with erythrocyte immunologic function have a good correlation.To the erythrocyte immunologic function,the detection is simple,the cost is chipe,The sesult is steay,it will be the better detection for immunologic function of esophageal carcinoma.
