兔关节内骨折错位关节软骨的自身修复 被引量:8

摘要 目的:研究并探讨不同程度纵向错位时关节运动功能的恢复及关节软骨损伤修复的形态学改变。方法:建立兔膝关节股骨内侧髁全厚(0.5mm)及两倍厚(1.0mm)关节软骨的骨折错位模型。在12周时,利用活体进行观测,并用光学显微镜和透射电镜对关节骨折错位的治疗和重建进行研究。结果:患膝关节运动功能,0.5mm错位较1.0mm错位恢复好。光学显微镜下,0.5mm错位组修复组织为类透明软骨;而1.0mm错位组修复组织则为纤维软骨。透射电镜发现,0.5mm错位较1.0mm错位修复组织软骨细胞成熟度高。结论:兔膝关节内骨折纵向错位不超过关节软骨厚度(≤0.5mm)时,软骨损伤自身修复效果满意。 AIM: To studied the effects of longitudinal fracture with step-off in different extent on healing of articular cartilaginous injury and repairing of articular motive function. METHODS: Two kinds of intra-articular step-off models of 0.5 mm and 1. 0 mm are established. At twelve weeks, the healing and remodeling of the intra-articular fracture with step-off defect are examined with observation of gross, LM and TEM. RESULTS: The repairing of articular motive function in 0.5 mm step-off animal group is better than that in 1.0 mm animal group. LM observation: After healing of intra-articular fracture, among all the experimental animal groups, repaired tissue is hyaline cartilaginoid in 0. S mm step-off group, but it is fibrous cartilage in 1. 0 mm step-off group. TEM observation: With 0. 5 mm step-off group, the chondro- cytes of the repaired is more developed, and the number of cellular organs is richer than that in 1.0 mm group. CONCLUSION: If the range of step-off defect is no more than the thickness of the articular cartilage(≤0. 5 mm), the healing of the cartilaginous injury is more satisfied.
