摘要
目的 观察血管紧张素转换酶抑制剂赖诺普利对慢性心力衰竭患者心室重塑和心功能的影响。方法 选择慢性心力衰竭患者 80例 ,随机分为对照组 (38例 )和治疗组 (42例 )。对照组采用综合疗法 ,治疗组在上述基础上加用赖诺普利 (5~ 10mg/d)。在治疗前和观察期满 6个月时分别测定心功能 (NYHA分级 ) ,心脏彩色超声测定左室结构及功能指标变化 ,测试 6min步行距离。结果 治疗组NYHA分级与治疗前相比明显改善 (P <0 0 0 1) ,治疗组与对照组比较左室射血分数上升[(5 1 1± 2 8) % ]vs[(34 2± 2 6 ) % ,P <0 0 0 1]、左室收缩末容积下降 [(118± 4 1 9)mLvs (15 3± 4 2 6 )mL ,P <0 0 0 1];左室舒张末容积与治疗前比较下降 [(16 2 5± 4 4 )mLvs (181 2± 4 5 0 ) ,P <0 0 5 ];患者 6min步行距离提高 [(372± 76 )mvs (341± 82 1)m ,P <0 0 5 ]。结论 赖诺普利可显著改善慢性心力衰竭患者心室重塑和心功能。
Objective To investigate the improvement of cardiac function and remodeling process of lisinopril on the patients with chronic heart faklure(CHF) Methods 80 CHF patients were divided into the control group(38 cases)and the treatment group(42 cases)randomly.The general therapy was given to the control group while Lisinopril(5~10 mg/d) was added to the treatment group besides general therapy.Before and after six-month period of treatment,the cardiac function(NYHA classification)was tested,and changes of function index of left ventricular structure were measured by color UCG and the distance of 6 minutes′ walk was measured.Result The cardiac function(NYHA classification)of the treatment group has been remarkably improved ( P <0 001), compared with the situation before treatment.The left ventricular ejecting fraction of the treatment group increased [(51 1±2 8)%vs(34 2±2 6)%, P <0 001],compared with the control group.The cubage dimension of the left ventricle at the end of constriction decreased [(118±41 9)mL vs (153±42 6)mL, P <0 001].The cubage dimension of the left ventricle at the end of the diastole decreased [(162 5±44)mL vs (181 2±45 0)mL, P <0 05],compared with the situation before treatment.The distance of 6 minutes'walk of the patients increased [(372±76)m vs (341±82 1)m, P <0 05].Conclusion Lisinopril could remarkably improve the ventricular remodeling and cardiac function of patients with chronic heart failure.
