摘要
目的 探讨桥本氏病核素显像时 ,甲状腺摄99mTcO4 量与腺体功能的关系 ,探索摄99mTcO4 量在桥本氏病中的临床应用价值。方法 对 49例健康对照者及细胞学穿刺确诊的桥本氏病患者 36例行甲状腺显像 ,将桥本氏病分为 :功能亢进组、功能正常组、亚临床甲减组、功能减退组 ,对摄99mTcO4 量与FT4进行相关分析 ,各组与对照组比较。结果 桥本氏病者摄99mTcO4 量与FT4呈正相关 (rs =0 .69,P <0 .0 0 1 )。功能亢进组、亚临床甲减组摄99mTcO4 量显著高于对照组 (P <0 .0 1 ) ;功能正常组、功能减退组与对照组无显著性差异 (P >0 .0 5)。功能亢进组摄99mTcO4 量均增高 ,功能正常组及亚临床甲减组摄取量可增多或正常 ,功能减退组可增多、正常或减低。摄99mTcO4 量减少者均属功能减退组。结论 桥本氏病者甲状腺摄99mTcO4 量与其功能变化间表现出复杂特点 ,摄99mTcO4
Objective To study the clinical value of the uptake 99m TcO 4 quantity (UQ) and the relationship between UQ and thyroid function when the patients with Hashimoto's disease (HD) were examined with 99m TcO 4 imaging.Methods Thyroid imaging was performed on 49 healthy controls and 36 HD that were finally diagnosed by cytological puncture.HD was divided into four groups:hyperfunction group,normalfunction group,subclinical hypothyroidism group, hypofunction group,and correlation analysis was performed between UQ and FT4,and each group was compared with control group.Results It was positive correlation(r s=0.69,P<0.001) between UQ and FT4 of HD,UQ of hyperfunction group and subclinical hypothyroidism group were significantly higher than that of control group (P<0.01);The difference between normalfunction group or hypofunction group and control group was not significant (P>0.05) ;UQ of every patient of hyperfunction group was high,and UQ of every patient of normalfunction group and subclinical hypothyroidism group was high or normal,and UQ of every patient of hypofunction group was high, normal or low. Patient whose UQ was lower than control group belonged to hypofunction group.Conclusion There are some complicated characteristics between UQ and thyroid function of HD;UQ possesses some values in the judgment of state of an illness and the diagnosis or differential diagnosis for HD.
