甲状腺/右上肺摄锝比值诊断甲状腺功能亢进的临床评价以及与甲状腺/唾液腺摄锝比值的自身对照 被引量:5

摘要 目的 评价甲状腺/右上肺摄锝比值(T/L)诊断甲状腺功能亢进(甲亢)的能力并与甲状腺/唾液腺摄锝比值(T/S)比较。方法 100例临床初诊怀疑甲亢,且排除口腔疾病和结节性甲状腺疾病者行甲状腺功能显像,并计算T/S与T/L。所有实验对象在甲状腺功能显像1周内明确诊断,45人被临床诊断为甲亢,55人甲状腺功能正常。结果100例受试者的T/S与T/L值具有高度相关性(r=0.937,P<0.001)。55例甲状腺机能正常人与45例甲亢患者T/L范围分别为2.0~8.5、10.1~103,二者之间没有重叠,诊断甲亢的灵敏度与特异性分别为100%。上述两组受试者T/S范围分别为1.1~8.7、4.5~38.1,有13%的受试者处于重叠范围。以重叠范围的中值作为诊断甲亢的下限,灵敏度和特异性分别为88.9%与94.5%。以临床诊断为对照,T/L与T/S诊断甲亢的正确指数有显著差异,T/L更准确。结论 T/L比T/S诊断甲状腺甲亢更为准确。 Objective To evaluate the accuracy of T/L (defined as radioactivity of thyroid over that of the same size in right upper lung on thyroid function imaging study) in diagnosis of hyperthyroidism and compare with T/S (defined as radioactivity of thyroid over that of salivary glands). Methods 99mTc thyroid imaging study were performed to calculate T/L and T/S in 100 patients with suspected hyperthyroidism, excluding nodular thyroid disease and oral disease. Clinical diagnosis was made within one week, 45 patients were diagnosed as having hyperthyroidism and 55 as having euthyroidism. Results High relationship was found between T/S and T/L values in these patients and significant difference was observed between Graves' disease and euthyroidism in both T/S and T/L values. There was no T/L value overlapping between Graves' disease and euthyroidism, with sensitivity of 100% and specificity of 100%; but there was some T/S value overlapping (13% of patients among this range), with sensitivity of 88.9% and specificity of 94.5%. Conclusion T/L value is more accurate than T/S in diagnosis of hyperthyroidism.
