摘要
目的 探讨慢性乙型肝炎患者HBV感染外周血淋巴细胞 (PBL)对HLA I类抗原的影响及在乙型肝炎慢性化机制中的作用。方法 36例慢性乙型肝炎 ,2 0例正常对照 ,用荧光定量PCR(FQ PCR)法测定PBLHBVDNA水平 ;用流式细胞术 (FCM)检测PBLHLA I类抗原的表达情况 ,并对PBLHBVDNA阳性患者HBVDNA含量与相应HLA I类抗原表达的相关性进行分析。结果 慢性乙型肝炎患者PBLHLA I类抗原的表达率 (% )和平均荧光强度 (MFI)均低于正常对照组 (P <0 .0 1) ;PBLHBVDNA阳性组与阴性组比较 ,HLA I类抗原的表达率和MFI显著降低 (P <0 .0 1) ;17例PBLHBVDNA阳性者的PBLHBVDNA含量对数值与相应HLA I类抗原表达率和MFI均呈负相关 ,相关系数分别为 - 0 .8893和 - 0 .86 89。结论 HBV感染PBL可能是慢性乙型肝炎患者外周血淋巴细胞HLA I类抗原表达降低的重要影响因素之一。
Objective To study the influence of peripheral blood lymphocytes (PBL) infected by hepatitis B virus(HBV) on the expression of HLA class I antigen in patients with chronic hepatitis B (CHB) and its effect on the pathogenesis of persistence of CHB.Methods Fluorescent quantitative polymerase chain reaction (FQ PCR) was applied to detect HBV DNA levels in PBL of 36 patients with CHB and 20 normal controls. The expression of HLA class I antigen on PBL were evaluated by flow cytometry (FCM). The correlation between the 2 parameters was also analyzed.Results 1.Both the expressive percentage and median fluorescence intensity (MFI) of HLA class I antigen in patients with CHB were all significantly lower than those in normal controls ( P <0 01).2.Both the expressive percentage and MFI of HLA class I antigen in the positive PBL HBV DNA patients group were all significantly lower than those in the negative PBL HBV DNA group ( P <0 01).3. Negative correlations between the logarithm of PBL HBV DNA concentrations and both expressive percentage and MFI of HLA class I antigen in 17 positive PBL HBV DNA patients with CHB were found. The correlation coefficients were -0.8893 and -0.8689 respectively.Conclusion HBV infected PBL may be one of important factors responsible for affecting the HLA class I antigen expression on PBL in the patients with CHB.
