肺癌血浆VEGF、血循环癌细胞及树突状细胞相互关系的研究

摘要 目的 分析非小细胞肺癌 (NSCLC)患者VEGF血浆含量与癌细胞血道转移及树突状细胞 (DC)成熟的关系。方法 外周血获自正常人、肺部良性疾患以及NSCLC患者。血循环中的癌细胞和DC按已建立的流式细胞仪定量分析技术进行检测。血浆VEGF含量采用ELISA方法测定。结果  12例正常人及 13例肺部良性疾患的血浆VEGF平均含量分别为 15 .7和 13.2 pg /ml,而 12 3例NSCLC患者的血浆VEGF平均含量为 140 .8pg /ml,显著高于正常人及肺部良性疾患者 (P <0 .0 0 1)。正常人及肺部良性疾患者外周血中癌细胞检测均为阴性 ,而 6 9例NSCLC患者血样中 32例阳性 ,阳性率为 46 .4% ,阳性患者的癌细胞平均含量为 0 .471× 10 6/L。正常人及肺部良性疾患者外周血中DC含量分别为 2 9.0和 2 2 .0× 10 6/L ,而 5 4例NSCLC患者的DC含量为 15 .9× 10 6/L ,显著低于正常人及肺部良性疾患者 (P <0 .0 5 )。若以VEGF≤ 5 6pg/m1作为正常值 ,则VEGF增高的NSCLC患者外周血中癌细胞阳性率及其含量明显增高而DC含量显著降低 ,其中以III、IV期患者最为突出。结论 NSCLC患者血浆VEGF增高具有促使癌细胞血道播散和抑制DC成熟的效应。 Objective To investigate the relationship between plasma level of vascular endothelial growth factor (VEGF), hematogenous dissemination of cancer cells and maturation of dendritic cells (DC) in patients with non-small cell lung cancer. Methods The peripheral blood was obtained from healthy subjects, patients with benign pulmonary diseases and with non-small cell lung cancers (NSCLC). The counts of DC and circulating cancer cells were quantified by flow cytometry according to the established methods. The plasma VEGF level was measured by ELISA kit. Results The average plasma VEGF levels of 12 healthy subjects and 13 patients with benign pulmonary diseases were 15.7 and 13.2pg /ml, respectively. The patients with NSCLC (123 cases) had a significantly increased VEGF level of 140.8 pg /ml ( P <0.001). Circulating cancer cells were found in 46.4% of NSCLC patients (32 /64 cases), and the average number of circulating cancer cells in these patients was 0.471×10 6 /L, but not in the other two groups. The count of DC in 54 patients with NSCLC(16.0×10 6 /L) was significantly decreased in comparison with those in healthy subjects (29.0×10 6 /L) and patients with benign lung diseases (22.0×10 6 /L), respectively ( P <0.05). Using 56 pg /ml of VEGF as normal value, it was found that NSCLC patients with elevated VEGF level had increased rates of positive test results and amounts of cancer cells in blood and much less counts of DC than those with normal VEGF level. Conclusion In patients with NSCLC, the increased plasma VEGF level appears to have the dual effects;i.e. promotion the of hematogenous dissemination of cancer cells and repressing the maturation of dendritic cells.
