摘要
目的探讨机械通气对重型颅脑损伤预后的影响。方法34例患者经吸氧不能改善缺氧且ALI指数<300,行机械通气治疗3～7d;机械通气前后行GCS评分,LIS评分,ALI指数及氧分压比较。结果治疗后上述各指标较治疗前均有显著改善。本组恢复良好及轻残23例,重残3例,植物生存2例,死亡6例。结论对吸氧不能改善缺氧的患者尽早采用机械通气,有利于意识恢复。
Objective To investigate the effect of mechanical ventilation on neurological outcome in the patients with severe brain injury.Methods Thirty-four patients with severe brain injury,in whom with ALI score<300hypoxemia was not improved after inspiring oxygen were mechanically ventilated for3～7days.The PaO 2 ,LIS,ALI score and GCS after the ventilation were compared with those before the veatilation in the patients with severe brain injury.Results The above parameter in the patients were significantly improved after the ventilation.Of 34patients received mechanical veatilation,23had a favorable neurological outcome,3had a poor neurological outcome,2were in a vegetative state,and6died.Conclusion The neurological outcome can be improved by early mechanical ventilation in the patients with severe brain injury,who suffered from pulmonary dysfunction.[
