丙泊酚联合阿托品与芬太尼用于无痛人流的临床观察 被引量:4

摘要 目的 :观察丙泊酚联合阿托品和芬太尼静脉麻醉用于人工流产术中的镇痛效果及安全性。方法 :将健康初次早期妊娠 ,要求人工流产的妇女 90例随机分成 3组 :第 1组 30例 ,术前单用丙泊酚静脉麻醉 ;第 2组 30例 ,术前先静脉推注阿托品 0 .5mg加枸橼酸芬太尼 0 .0 5mg ,然后静脉推注丙泊酚 ;第 3组 30例不用药。比较 3组人工流产术疼痛程度、宫颈松驰度、术中出血情况、丙泊酚用量及不良反应。结果 :第 1、2组有显著镇痛效果 ,第 2组的丙泊酚总用药量比第 1组明显少 ,两组用药后 ,血压、心率均下降但在正常范围。第 2组宫口松驰例数明显多于第 1、3组。 90例患者无 1例发生严重毒副反应。结论 :丙泊酚联合阿托品及芬太尼开展无痛人流安全有效 ,不仅减少了丙泊酚用量 ,提高了经济效益 。 Objective: To observe the painless effect and security during the induced abortion by means of vein anesthesia of propofol combined with atropine and sublimaze.Methods:90 healthy women in their first and early gestational period were divided into three groups with 30 in each group.Before operation, those in group 1 were only given vein anesthesia of propofol; in group 2 intravenous injection of atropine 0.5 mg and sublimaze 0.05 mg followed by propofol;in group 3 no drugs were given.Pain free effect,the flabby of uterine neck,the bleeding during the operation, the dose of propofol and its bad reaction were determinated.Results:Although the dose of propofol used in group 2 was smaller than that in group 1,the pain killing effect were both obvious.Moreover,both the blood pressure and the rhythm of the heart of those in the two groups declined but within the normal range.The result also showed that there were more with flabby of uterine neck in group 2 than that in the other two groups and that group 2 was free from poisonous effect.Conclusion:The method of painless induced abortion with propofol combined with atropine and sublimase is a safe and effective one with little side effect, not only reducing the dose of propofol but increasing the profit.
