体外和非体外循环冠状动脉搭桥术对肺组织的影响 被引量:2

Off-pump versus on-pump coronary artery bypass grafting:Effects on pulmonary structure

摘要 目的 :研究体外循环和非体外循环冠状动脉 (冠脉 )搭桥术对肺组织的影响。方法 :2 0例接受选择性冠脉搭桥术的患者分为 2组 ,A组 (10例 )为体外循环下冠脉搭桥组 ,B组 (10例 )为非体外循环下冠脉搭桥组。检测各组血清弹性蛋白酶、白细胞介素 8(IL 8)和肿瘤坏死因子 α(TNF α)的浓度 ,分析肺组织损伤的参数。结果 :A组弹性蛋白酶术后 1h、IL 8术后 6h、TNF α在术后 2h达到最高值 (P <0 .0 1)。A组肺活检组织的丙二醛含量显著高于B组 (P <0 .0 5 )。病理切片检查示A组有肺组织损伤表现 ,A组所有的参数在术后 2 4h趋于正常。结论 Objective:To investigate the effects on lung in patients undergoing coronary artery bypass grafting with cardiopulmonary bypass (on pump CABG) and those without cardiopulmonary bypass (off pump CABG). Methods:Twenty patients undergoing elective CABG were included; 10 patients (group A) underwent on pump CABG and 10 patients (group B)underwent off pump CABG. Parameters of lung injury were studied, as well as plasma levels of elastase, IL 8 and TNF α. Results:In group A, the level of elastase reached highest 1h after surgery, the level of IL 8 reached the highest 6 h after surgery, while the TNF α did it 2 h after surgery, P< 0.01 . The level of MDA in lung biopsy tissue was significantly higher in group A than that in group B. The lung biopsy tissue showed some signs of injury in group A. There was no significant change in group B. However, concentrations of all these parameters in group A returned to normal 24 h after surgery. Conclusion:Signs of oxidative stress and injury of lung tissue are significant on pump CABG as compared with that of off pump CABG.
