摘要
目的 :观察带蒂颞肌筋膜瓣对几丁质室内兔颞骨内面神经再生的影响。方法 :选用 40只成年日本大耳白兔 ,横断颞骨内面神经后 ,用几丁质管桥接形成人工室 ,实验组人工室外包绕带蒂颞肌筋膜瓣 ,对照组未包绕 ,动物均左耳为手术侧。术后 3个月和 5个月分别进行大体观察、电生理测试、组织学观察和形态学分析。结果 :同期颞肌筋膜包绕组与对照组比较 ,再生神经诱发电位的潜伏期缩短 ,振幅增高 ,再生有髓神经纤维直径增粗 ,髓鞘增厚。结论
Objective: To observe whether a well vascularized temporoparietal fascial flap would improve rabbit intratemporal facial nerve regeneration repaired with chitin chamber.Methods:Defects(6mm) of left intratemporal facial nerve were created in 40 rabbits and were repaired with chitin chamber.In the experimental group,the chitin chamber was wrapped in pedicled temporoparietal fascial flap,whereas in the control group it was not.The gross observation,electrophysiological test,histological examination and morphological analysis were performed at 3 and 5 months after operation.Results:In the same perioed after operation,incubation period of evoked potential was shorter and amplitude higher in the experimental group.Fibrous diameter of the regenerated nerve was larger and myelin sheath thicker in the experimental group as well.All of them were statistically different in comparison with the control group.Conclusion:This study suggested that pedicled temporoparietal flap could improve intratemporal facial nerve regeneration within chitin chamber.
