摘要 目的 探讨缺血性脑血管病患者血清血管内皮细胞生长因子 (VEGF)的动态变化及其临床意义 ,以及与白细胞介素 1β(IL 1β)的关系。 方法 采用双抗体夹心 (ELISA)法和平衡放射免疫法分别测定 38例脑梗死 (CI)患者发病 2 4小时内、3天、7天、14天和 16例短暂性脑缺血发作 (TIA)患者 2 4小时内及 30名健康对照者的VEGF及IL 1β浓度。结果 脑梗死组在病程各时间点上血清VEGF浓度与TIA组及对照组相比均增高 (P <0 .0 1) ,且呈动态变化 ,于第 7天达高峰 ,第 14天时仍未恢复正常 ;大梗死灶患者VEGF水平升高明显。 2 4小时内脑梗死及TIA患者IL 1β浓度与对照组相比明显增高 (P <0 .0 1) ,IL 1β水平在脑梗死组病程中亦呈动态变化 ,于 2 4小时内达高峰 ,随后下降 ,第 7天降至正常。各时间点IL 1β与VEGF浓度间未发现有相关性 (均P >0 .0 5 )。结论 VEGF及IL 1β参与了缺血性脑损伤的病理过程。 Objective To study the dynamic changes and clinical significance of vascular endothelial grouth factor(VEGF) and Interleukin 1β(IL 1β) in patients with ischemic cerebral vascular disease.Methods Serum VEGF and IL 1β levels were determined by ELISA and balanced radioimmunoassay respectively in 16 patients with TIA and 38 patients with cerebral infarction during different phase (in 24 h,at 3 d,7d and 14 d after the onset of disease),and 30 healthy controls were included.Results It was found that the levels of VEGF in patients with infarction were much higher than those with TIA( P < 0.01 ) and control groups with a highly significant difference,the levels of Serum VEGF gradually increased after the onset of acute cerebral infarction and reached the peak at 7 d,and didn't recover normal at 14 d.We alson found that the infarction patients with larger focus had higher VEGF levels.The levels of IL 1β in acute infarction and TIA groups were higher than those in control group( P < 0.01 ).The level in 24 h was the highest,and decreased gradually later.No correlation was found between the levels of VEGF and IL 1β at different phase of ischemic infarction( P > 0.05 ).Conclusion VEGF and IL 1β may take part in the pathological process of ischemic brain damage VEGF may serve as a sensitive monitor to estimate the cerebral ischemic degree and clinical prognosis.
