新生儿缺氧缺血性脑病心肌酶、肾功能及电解质的改变与临床意义

摘要 目的:检测中、重度新生儿缺氧缺血性脑病(hypoxic ischemic encephalopathy,HIE)心肌酶、肾功能和电解质,并分析其临床意义。方法:检测40例中、重度窒息新生儿急性期、恢复期的心肌酶谱(AST、LDH、CK、CK-MB、HBDH)、肾功能(BUN、Cr)和血清Na^+、K^+、Cl^-、Ca^(2+)、Mg^(2+),并与30例正常新生儿对照比较。结果:中、重度新生儿HIE急性期心肌酶谱每项均高于对照组,差异显著(P<0.01)。中、重度组血清BUN、Cr均高于对照组,血清Ca^(2+)、Mg^(2+)中、重度窒息组显著低于对照组。结论:新生儿缺氧缺血性脑病,除有脑损伤外,常伴有心肾功能异常、电解质紊乱。监测和治疗心肾功能损害,对提高成活率,减少后遗症十分重要。 Objective: To detect myocardial enzyme spectrum, renal function and electrolytes of the moderate and severe neonatal hypoxic-ischemic encephalopathy (HIE) and analyse the clinical significance. Method: Myocardial enzyme spectrum (AST, LDH, CK, CK-MB, HBDE), renal function (BUN, Cr), and serum electrolytes (Na+、 K+、 Cl-、 Ca2+、 Mg2+ ) of 40 neonates with moderate and severe asphyxia were detected in acute and recovery phases and compared with normal ones. Result: Items of the myocardial enzyme spectrum of the moderate and severe asphyxia neonates had a higher level than that of the control group and the difference was obvious (P < 0.01), So is the renal function. But the level of the serum Ca2+ , Mg2+ is obviously lower than the control group. Conclusion: Besides brain damage, disfunctions of the heart and kidney occur also in HIE, therefore , detecting the myocardial and renal functions and prompt treatment are very important to decrease the incidence of the sequela and increase survival rate.
